The relationship between HLA B27 allele and degenerative disease of lumbal spine

Abstract:

Alela HLA-B27 je součástí Hlavního histokompatibilitního komplexu, který zodpovídá za imunitní odpověď organismu. Tato alela je známá zejména svou asociací s různými autoimunitními revmatickými onemocněními a nejvíce je zastoupena u ankylozující spondylitidy (AS). K dalším degenerativním onemocněním, které postihují páteř, patří vedle AS také vyhřeznutí meziobratlové ploténky. V různých studiích bylo prokázáno, že je toto onemocnění ovlivněno nejen těžkou prací, ale také imunologicky. Našim cílem tedy bylo prokázat, že alela HLA-B27, která je spojena ve velké míře s různými autoimunitními chorobami, ovlivňuje i vyhřeznutí meziobratlové ploténky. Soubor osob tvořilo 140 pacientů mužského pohlaví, u kterých bylo potvrzeno vyhřeznutí meziobratlové ploténky a 86 kontrolních pacientů mužského pohlaví, kteří se léčili kvůli jiným onemocněním. Ze 140 pacientů bylo pozitivních na alelu HLA-B27 25 osob a z kontrolních pacientů 11 osob. Výsledky byly vyhodnoceny dvoustranným Fisher exact testem, který vyloučil vztah alely HLA-B27 k výhřezu meziobratlové ploténky.

Abstract:

Allele HLA-B27 is part of a Major histocompatibility complex, that is responsible for the immune response of an organism. The allele is known especially for its association with various autoimmune rheumatic diseases and mostly it is represented mainly in ankylosing spondylitis (AS). Between other degenerative diseases that affect spine, also belongs, as well as AS, the intervertebral disc prolapse. Various studies have shown that the disease is caused not only by hard work, but also by immunity system. Our aim was therefore to prove the relationship between intervertebral disc prolapse and HLA-B27 allele, that is largely associated with various autoimmune diseases. Testing group comprised of 140 male patients with confirmed intervertebral disc prolapse and 86 male control patients that were treated for other diseases. 25 patients and 11 controls were positive for HLA-B27 allele. Results were evaluated by bilateral Fisher exact test. Test negated HLA-B27 relation with the intervertebral disc prolapse.
