Comparison of midwifery in Czech Republic and Estonia

Abstract:

Téma: "Komparace porodní asistence v České republice a Estonsku". Cíl: Hlavním cílem mé bakalářské práce je porovnat rozdíly porodní asistence v České republice a Estonsku. Metoda: Celá práce je čistě teoretická, pro získání potřebných informací bylo čerpáno především z odborné literatury, zákonů, vyhlášek a směrnic Evropské Unie a daných zemí, z oficiálních webových stránek organizací porodních asistentek a některých vysokých škol, kde je vyučován obor porodní asistence, z oficiálních příruček a průvodců, z webových stránek některých soukromých porodních asistentek, z oficiálních webových stránek některých porodnic a též z vlastních zkušeností v souvislosti s absolvovanou praxí v Masarykově nemocnici v Ústí nad Labem a v Pelgulinna Sünnitusmaja a Ida-Tallinna Keskhaigla v Tallinnu. Výsledky: Kompetence porodních asistentek v České republice a Estonsku jsou prvotně závislé na směrnici Evropské unie. Tyto jsou v daných zemích téměř shodné, jen v Estonsku je kompetence porodních asistentek rozšířena o podávání některých léků bez ordinace lékaře. V obou zemích musí budoucí porodní asistentky tento obor studovat na vysokých školách, kde získají titul Bc. V České republice toto studium činí tři roky, v Estonsku čtyři a půl roku. Obě země vycházejí ze směrnice Evropské unie, jejich teoretická i praktická náplň výuky je téměř shodná. V Estonsku jsou ve zdravotnických zařízeních kompetence porodních asistentek více brány v potaz, v České republice je i za fyziologický porod, který fakticky vedla porodní asistentka, vždy zodpovědný lékař. Péče soukromých porodních asistentek v ani jedné ze zemí není hrazena ze zdravotního pojištění. V Estonsku momentálně neexistuje žádná soukromá porodní asistentka, která by nabízela péči ženám během těhotenství, ve zdravotnických zařízeních tuto péči naopak poskytují výhradně porodní asistentky. V České republice tuto péči zajišťují buď sami lékaři, nebo soukromé porodní asistentky. V obou zemích k výkonu této profese musí zájemce splnit studijní podmínky a poté se registrovat na příslušném místě. Obě země mají též profesní organizace porodních asistentek. Závěr: Určité rozdíly mezi porodní asistencí v České republice a Estonsku jsou, obě země však vycházejí ze směrnic EU a organizace porodních asistentek v obou zemích mají shodné cíle a obdobnou vidinu budoucnosti porodních asistentek.

Topic: A comparison of midwifery in the Czech Republic and Estonia Objective: The main objective of this work is a comparison of differnces of midwifery between the Czech Republic and Estonia. Methodology: This work is theoretical, in order to obtain the necessary informations have been used specialised literature, laws, decrees and directives of EU and both countries, the official websites of the organisations of midwifes and some universities, where midwifery is taught. Furthermore have been used official handbooks and guides, official websites of obstetric departments. The work is also build on my own experiences gained during my practical training in Masarkoyva nemocnice Ústí nad Labem and in Pelgulinna Sünnitusmaja and Ida-Tallinna Keskhaigla in Tallinn. Results: Midwife´s competences in the Czech Republic and Estonia are build on European Union directive. This competences are almost identical in both countries. The main difference is that in Estonia midwifes can also administer some kind of medication without medical prescription. After the studies midwifes have to complete, they get bachelor degree in both countries. In the Czech Republic the study takes three years, in Estonia it takes for and half years. Both countries are based on European Union directive, the theoretical and practical training is almost identical. The midwife´s competences in Estonian hospitals are more covered than in the Czech Republic, where doctor is always responsible for all of childbirths, even though midwife cared for woman. In no country the midwifery private care is covered by the public health care insurance system. In Estonia there is no private midwife who offers her care to pregnant women, on the contrary in hospitals solely midwifes care for pregnant women. In the Czech Republic doctors or private midwifes provide the same custody to women. A person who wants to make this profession has to meet the studying conditions and after that make a registration in the competent authority. In both countries there are also organizations of midwifes. Conclusion: There are some differences between midwifery in the Czech Republic and Estonia. The legislative in the countries is based on European Union directive and the organizations of midwifes in this countries have identical objectives and similar vision of the midwife´s future.
