The state of dental health of children in relation to their regimen - an international comparison

Abstract:

Úvod. Zubní kaz je nejrozšířenější chorobou multifaktoriálního původu. Vede k destrukci zubu, bolesti, až ztrátě zubu. Následky jsou nejen místní, ale i celkové. Cílem studie bylo porovnat stav zubního zdraví dětí žijících v geograficky, ekonomicky a kulturně rozdílných zemích - středně ekonomicky vyspělé České republice (ČR) a rozvojové zemi - Jemenu (YE). Soubor a metodika. Bylo vyšetřeno 875 dětí (věk 5 – 15 let). Děti byly rozděleny do 4 věkových skupin a dle pohlaví. Pro výpočet u dočasného chrupu byly použity indexy kpe a restorativní index ri % a u trvalého chrupu indexy KPE a RI %. Ke statistickému vyhodnocení byl použit software MedCal Software inc. Belgium. Statistické vyhodnocení bylo provedeno pomocí t-testu, Pearsonovy korelace, korelačního koeficientu (rho) s intervalem spolehlivosti (95% CI), Spearmanův rank korelační koeficient s 96% CI. Jako statisticky významné byly považovány hodnoty p < 0,05. Výsledky. V průměrných hodnotách kpe a KPE/jedno dítě se většinou výstupy statisticky významně nelišily v průměrných hodnotách kpe a KPE / jedno dítě. Z údajů ri % a RI % bylo zjištěno, že v Jemenu nebyly sledované děti vůbec ošetřovány a v České republice bylo procento ošetřených zubních kazů neuspokojivé. Jako statisticky významné se ukázaly četné pozitivní korelace kpe a KPE. Frekvence dětí bez zubního kazu se pohybovala mezi 5 - 19 % u nejmladších dětí (věk 5 – 9 let) a 4 – 11 % u dětí starších (věk 10 – 15 let); v obou zemích hluboko pod hodnotou doporučovanou WHO. V antropometrických údajích se děti z obou zemí ve všech věkových kategoriích téměř vždy statisticky významně lišily. Děti z ČR byly vyšší, s větší tělesnou hmotností i hodnotami BMI. Neprokázali jsme, že naznačené pozitivní korelace mezi BMI a indexy kpe/KPE by byly statisticky významné. Závěr. Výsledky hodnocené podle obecně používaných indexů kpe/KPE ukázaly, že stav zubního zdraví u dětí v České republice a dětí z Jemenu se příliš neliší. Nicméně je evidentní, že v České republice je stav významně lepší, i když zdaleka ne výborný.

Abstract:

Introduction: Tooth decay is the most prevalent disease of multi factorial origin, which results to destruction of the tooth resulting in pain and tooth loss. Since the disease is almost entirely preventable and easily curable at the beginning, the extension of tooth decay reflects the level of population health literacy and accessibility of professional dental care. The aim of this study was to compare the quality of dental health of children living in geographically, economically and culturally very different countries: the economically medium developed country of Czech Republic (CR) and Yemen (YE), which is an underdeveloped country suffering from civil war since years. Material and methodos. The study by standard methods examined 875 children aged 5-15 years. For comparing the data between the two countries, the children were divided into four groups each representing both sexes (girls and boys). The calculated standard indexes: kpe and degree of dental treatment restorative index ri% for deciduous teeth and KPE and RI% for permanent dentition were used. For the statistical evaluation of differences we used software from MedCalc Software inc., Belgium, specifically the t-test, Pearson correlation, the correlation coefficient (rho) with a confidence interval (95% CI), Spearman rank correlation coefficient for the correlation kpe-KPE with 96% Cl. Statistically significant p values were considered the ones of less than 0.05. Results. The commonly used indexes kpe / KPE per child applied to individual child showed that the children in the Czech Republic and children in Yemen are not much statistically different. Data ri% and RI% indicates that monitored children in Yemen were not medically treated at all and the percentage of restored teeth decay in Czech Republic is not sufficient. As statistically significant were valued results which showed a positive correlation kpe and KPE. The frequency of children without teeth decay varied between 5-19% among the youngest age groups and 4-11% among older children, which is in both countries deeply under the levels recommended by WHO mentioned among global goals by the WHO for the second decade of the 21st centiry. In all age groups in both countries the children almost always differed statistically significantly. According to antropometry data the children from the Czech Republic were higher, with higher body weight and BMI values. We did not prove that indicated positive correlations between BMI and indexes kpe/KPE were statistically considerable. Conclusion: The results suggest that the level of dental health of children from Czech Republic and Yemen are usually not significantly different in levels of kpe and KPE. However, it is evident that the quality of dental care in the Czech Republic significantly improved but it is still far from being satisfactory.
