Comparison between a physiotherapy rehabilitation treatment protocol and a self-recovery protocol after subastragalar arthrorisis surgery.

Abstract:

The subtalar arthrorisis operation is that surgical treatment aimed at correcting the so called pronatory syndrome ( or flat foot in children age), which is represented by the pathological condition that involves the flattening of the medial longitudinal arch coupled with the valgus rear foot, not solving with conservative treatments (orthosis and kinesiotherapy). The operation consists in the introduction of an endorthesis via percutaneous inside the sinus tarsi. The sinus tarsi is that channel existing between calcaneous and talus, placed outwardly the foot, at the level of subtalar joint whose extreme excursion in pronation is responsible for the deformation. Therefore the introduction of a slotting screw in this place (subtalar arthrosis), or screwed in calcaneous so that the head sticks out on the sinus pavement, limits the hyperpronation thus avoiding the sliding of the talar on the calcaneous. The following step, after this procedure, is the immobilization for about two weeks of the foot through a plastered knee sock and it is necessary for the patient not to overload the operated foot. Against this intervention, during post-surgery, the structure where I developed this study involves two different protocols: the first one prescribes ten physiotherapeutic sessions to the patient, that leads to the functional recover of the operated foot and to the right walking. The second protocol, instead, guarantees the patient a normal recovery of functionality without needing any rehabilitative treatment (protocol of non-treatment). L?intervento di artrorisi sottoastragalica č quella procedura chirurgica volta alla correzione della cosiddetta sindrome pronatoria ( o piede piatto in etŕ evolutiva), ossia quella condizione patologica nella quale si ha l?appiattimento della volta plantare a cui si associa un valgismo del retro piede, non correggibile tramite trattamenti di tipo conservativo (ortesi e chinesiterapia). L?intervento consiste nell?introduzione di una endortesi per via percutanea all?interno del seno del tarso. Il seno del tarso č quel canale normalmente esistente tra calcagno e astragalo, localizzato esternamente al piede, a livello dell?articolazione sottoastragalica la cui escursione eccessiva in pronazione e? responsabile della deformitŕ. L?introduzione di una vite ad incastro in questo spazio (artrorisi subastragalica), o avvitata nel calcagno in modo che la testa sporga nel pavimento del seno (calcaneo stop), limita l?iperpronazione impedendo lo scivolamento dell?astragalo sul calcagno. In seguito a tale procedura, si procede con l?immobilizzazione del piede attraverso gambaletto gessato, per un periodo di circa 2 settimane, durante il quale il paziente osserva la completa astensione dal carico sul piede operato. A fronte di tale intervento, nel post-operatorio, la struttura nella quale ho realizzato questo studio, prevede l?utilizzo di due differenti protocolli: il primo prescrive al paziente 10 sedute di fisioterapia, volte al recupero funzionale del piede operato e alla corretta deambulazione.
