Monitoring of locomotion parameters during amputee walking school

Abstract:

Bakalářská práce se zabývá problematikou chůze u jedinců amputovaných na dolní končetině. Práce je rozdělena na dvě části, teoretickou a praktickou. V teoretické části jsou popisovány důvody a typy amputací dolních končetin, možnosti protetického řešení a následně je popisován proces vedoucí k reedukaci chůze a problematické aspekty chůze s protézou. V praktické části je popsáno šest respondentů, na kterých bylo prováděno měření parametrů chůze pomocí přístroje Bionic Pro. Tři z nich byli amputováni v oblasti stehna a tři v oblasti bérce. Respondenti byli měřeni v rámci čtrnáctidenního rehabilitačního pobytu, na začátku pobytu a následně na jeho konci. Cílem práce bylo zjistit, jestli v rámci rehabilitačního pobytu došlo ke změně parametrů chůze u zúčastněných respondentů a jak se lišily parametry chůze u respondentů se stehenní a bércovou amputací. Konkrétně se práce zabývala dvěma výzkumnými otázkami. Zaprvé, jak se změnily parametry stojné fáze u respondentů po absolvování rehabilitačního pobytu a zadruhé, jak se lišily výstupní naměřené hodnoty podklesávání kontralaterální poloviny pánve během stojné fáze na homolaterální polovině pánve ve frontální rovině mezi respondenty se stehenní a bércovou amputací. V rámci diskuze byly porovnávány zkoumané hodnoty a bylo zjištěno, že zatímco první výzkumná otázka se shodovala svými výsledky i s dalšími studiemi, tak druhá výzkumná otázka odhalila značně odlišné závěry než studie zkoumající podobné aspekty. Hodnoty stojné fáze u respondentů na konci pobytu se vesměs zlepšily a přiblížily se hodnotě ideální normy stojné fáze, ale hodnoty podklesávání pánve byly nižší při stojné fázi na protéze u respondentů amputovaných v bérci než u těch amputovaných ve stehně, což se rozcházelo s výsledky jiných studií. Je zapotřebí provedení dalších výzkumů věnujících se problematice chůze u amputovaných. Je doporučeno přístroj Bionic Pro podrobit více odborným studiím.

Abstract:

The bachelor thesis deals with the problem of gait in lower limb amputees. The thesis is divided into two parts, theoretical and practical. The theoretical part describes the reasons and types of lower limb amputations, the possibilities of prosthetic solutions and then describes the process leading to gait re-education and the problematic aspects of walking with a prosthesis. The practical part describes the six respondents on whom gait parameters were measured using the Bionic Pro device. Three of them were amputees in the thigh region and three in the tibia region. The respondents were measured during a 14-day rehabilitation stay, at the beginning of the stay and then at the end of the stay. The aim of the study was to determine whether there was a change in gait parameters in the participating respondents during the rehabilitation stay and how the gait parameters differed between the respondents with femoral and tibial amputation. Specifically, the thesis addressed two research questions. Firstly, how did the standing phase parameters change in the respondents after completing the rehabilitation stay and secondly, how did the baseline measurements of contralateral pelvic flooring during the standing phase on the homolateral half of the pelvis in the frontal plane differ between respondents with femoral and tibial amputation. In the discussion, the examined values were compared and it was found that while the first research question was consistent in its results with other studies, the second research question revealed significantly different findings than studies examining similar aspects. The standing phase values of the respondents at the end of the stay were universally improved and approached the ideal standing phase norm, but the pelvic buckling values were lower during standing phase on the prosthesis for respondents amputated at the tibia than for those amputated at the femur, which was inconsistent with the results of other studies. Further research addressing the issue of gait in amputees is required. It is recommended that the Bionic Pro be subjected to more expert studies.
