Abstract:

SOJKA, Michal. Terminální stavy v přednemocniční péči. Vysoká škola zdravotnická, o.p.s. Stupeň kvalifikace: Bakalář (Bc.). Vedoucí práce: MUDr. Natália Podstavková. Praha. 2022. 62 stran. Tématem bakalářské práce jsou terminální stavy v přednemocniční péči. V teoretické části jsou definovány základní pojmy jako jsou terminální stav, smrt a umírání, specifika paliativní péče a její druhy, dále jsou vymezeny nejčastější symptomy u pacientů v paliativní péči, včetně možných řešení. Závěrečná kapitola teoretické části je věnována profesi zdravotnického záchranáře a jeho kompetencím dle legislativy České republiky. Cílem praktické části je zmapování aktuální situace v přednemocniční péči u pacientů v terminálním stádiu onemocnění. Byla zvolena metoda kvalitativního výzkumu formou šesti polostrukturovaných rozhovorů s lékaři a záchranáři pracujícími na ZZS. Z odpovědí dotazovaných vyplývá, že se jedná o aktuální téma a postupy v přednemocniční péči u této skupiny pacientů nelze jednoduše zevšeobecnit. Tato práce poukazuje na největší problémy v této oblasti z pohledu pracovníků na ZZS a přináší nápady na možné řešení a zlepšení této situace. Péče o paliativní pacienty je nedílnou součástí výjezdu ZZS, a proto je důležité tuto problematiku zařadit do vzdělání a běžné praxe pracovníků poskytujících přednemocniční neodkladnou péči.

The topic of this bachelor thesis are terminal states in pre-hospital care. The theoretical part of the thesis is focused on the definitions of a terminal state, death, dying and the palliative care. The palliative care is the main objective of the thesis in terms of types and specifics as well as the most common symptoms of patients in the palliative care and possibilities of their treatment. The last part is dedicated to the professional competency standards of paramedic according to the Czech legal system. The aim of the thesis is to convey the current situation in the palliative care in the terminal states of patients´ illness. The methodology of qualitative research was applied in six semi-guided interviews with medical doctors and paramedics employed at Emergency Medical Service of the South Moravian region. The interviews reveal that medical professionals who took part in the research concluded that the palliative care has recently become an issue of great importance. Moreover, the results indicate that it is not possible to apply one general treatment in pre-hospital palliative care to all kinds of patients. Thesis focuses on the most difficult issues in pre-hospital palliative care from the paramedics´ perspective. The purpose of the thesis is to provide innovative ideas in approaching pre- hospital palliative care as well as to suggest the ways of improving the current situation. The palliative care plays an integral role in the treatment provided by Emergency Medical Service. It is very important to implement education about pre-hospital palliative care into education so that it can be later used in everyday practice of medical professionals serving in the emergency rescue system.
