Multidisciplinary management of an elderly patient with a femur fracture in a clinical presentation of polyneuropathy in the four limbs: a clinical case

Abstract:

The term neuropathies refers to a degeneration of nervous tissue involving the nerves of the snp, a condition that can be transient or permanent depending on the disturbe. Neuropathies are divided into axonals, where the axon is injured, mainly for metabolic problems with dismal-proximal onset, and demielinizing that is, at the expense of the myelin sheath with reduced the speed of nerve conduction. In this case study I present the case of a 68-year-old man hospitalized following a fall with pretrocanteric fracture to the left femur and subjected to osteosynthesis with PCCP plaque; the patient presents a total four limbs polineuropathy. After an extensive evaluation we set a treatment plan of 2 hours a day 5 days a week. The first goal of our rehab program is the recovery ot the hip's ROM, then we work on the walking in the room and accustomed to the postural steps. The treatment is so divided: morning strengthening with lifts to the shoulder pad and walking at the parallels first holding with two hands later with one, in the afternoon builiding muscle to cot with rubber bands, wearable weights static stretching and P.I.R. Subsequently is carried out with crutches, first two then one and is taught to properly climb the stairs, a program of muscle reinforcement is maintained and inserted obstacles that simulate situations. The patient walks autonomously with the stick, then is analyzed and simulated the throwing movement in the bowls with a sponge ball and the weight of 1 kg. The patient reacted very positively to the treatment and returned to ADL using the stick as an aid.

Abstract:

Con il termine neuropatie si indica una degenerazione del tessuto nervoso che coinvolge i nervi del sistema nervoso periferico, condizione che pu? essere transitoria o permanente a seconda del disturbo. Le neuropatie si dividono in assonali, dove viene lesionato l'assone, principalmente per problematiche metaboliche con esordio disto-prossimale, e demielinizzanti cio? a carico della guaina mielinica con riduzione della velocit? di conduzione nervosa. In questo elaborato tratteremo il caso di un uomo di 68 anni ricoverato a seguito di una caduta con frattura pertrocanterica al femore sinistro e sottoposto ad osteosintesi con placca PCCP con un quadro pregresso di neuropatia ai 4 arti. Dopo attenta valutazione si imposta un piano di trattamento di 2 ore al giorno 5 giorni a settimana. Il primo obiettivo ? stato dapprima il recupero del ROM articolare dell'anca, successivamente viene fatto deambulare nella stanza e abituato ai passaggi posturali. Il trattamento ? cos? suddiviso: mattino rinforzo con alzate alla spalliera e deambulazione alle parallele con crescente difficolt? nel pomeriggio rinforzo muscolare a lettino con elastici, pesi indossabili, stretching statico e P.I.R. Successivamente viene fatto deambulare con le stampelle, prima due poi una e gli viene insegnato a salire correttamente le scale, viene mantenuto un programma di rinforzo muscolare ed inseriti degli ostacoli che simulano situazioni di vita quotidiana da superare. Il paziente deambula autonomamente con il bastone, quindi per tornare alla sua consueta partecipazione viene rintrodotto e simulato il movimento di lancio nelle bocce. Il paziente ha reagito molto positivamente al trattamento ed ? tornato alle ADL utilizzando il bastone come ausilio.
