Snížení bolestivosti artrotických kyčelních kloubů vlivem fyzioterapeutických postupů a redukce hmotnosti

Physiotherapy and weight loss as a treatment of coxarthrosis

Abstract:

Metodika: Ve své práci jsem použil metodiku rozhovoru ke sběru informací, dále pozorování, testování a měření. Zvolil jsem klientku s II. stupněm artrózy pravého kyčelního kloubu. V rámci kineziologického rozboru byly použity následující metody vyšetření: vyšetření aspekcí (držení, konfigurace, trofika - antropometrie), vyšetření palpací (hybnost - goniometrie, vyšetření pohyblivosti páteře, svalový tonus, svalová síla, vyšetření zkrácených struktur, pohybových stereotypů, kloubní vůle, fascií a trigger pointů), dále funkční vyšetření (Trendelenburgova zkouška, Patrickovo znamení) a dynamické vyšetření (stereotyp chůze a její modifikace). V neposlední řadě byla vyšetřena bolestivost pravého kyčelního kloubu dle číselné škály intenzity bolesti. V terapii jsem použil techniky měkkých tkání, míčkovou facilitaci, postizometrickou relaxaci, postfacilitační strečink, izometrickou kontrakci, Dynamickou neuromuskulární stabilizaci, Brügger koncept s využitím Thera-Bandu. Na základě výsledků jsem stanovil krátkodobý plán fyzioterapie, který trval 5 týdnů. Výsledky: Došlo ke zvětšení rozsahu pohybu na pravém kyčelním kloubu do vnitřní rotace o 5° a abdukce o 10°. Na pravé dolní končetině vzrostla síla do abdukce v kyčelním kloubu o 1 stupeň. Dále došlo ke snížení klidové bolesti pravého kyčelního o 3 stupně hodnocené číselnou škálou intenzity bolesti. Podařilo se odstranit patologickou bariéru v oblasti inguinální fascie, dále došlo ke zlepšení stereotypu abdukce v kyčli bilaterálně, ke snížení zkrácení m. piriformis a m. iliopsoas o 1 stupeň. Klientka redukovala 10 kg tělesné hmotnosti z 97,3 kg na 87,3 kg. Závěr: Během terapií s mojí klientkou se povedlo dosáhnout cílů, které jsme si před intervencí stanovili, zejména se snížila bolestivost, což mělo vzhledem k její diagnóze pozitivní vliv na její psychiku. Klientka je motivována k dodržování stanovených doporučení i nadále.

Abstract:

Methodology: In my work I use methodical interviews to gather information, as well as observation, testing and measurement. I chose a patient with II. degree of osteoarthritis of the right hip. Within the kinesiological analysis, which includes examination of aspects (posture, configuration, trophy), palpation examination (examination of spinal mobility, muscle tone, muscle strength, examination of shortened structures, movement, functional examination (Trendelenburg test, Patrick's sign) Dynamic examination (stereotype of gait and its modification) and examination of the numerical scale of pain.In therapy I use soft tissue techniques, ball facilitation, PIR, PFS, IC, Dynamic neuromuscular stabilization, Brügger's concept using Thery-Band. Based on the results I set a long-term plan physiotherapy. Results: There was an increase in the range of motion on the right hip to internal rotatino 5 ° and abduction by 10 °. On the rgiht leg, the muscle strenght to abduction on the right hip increased by 1 degree. There was also a reduction in resting pain by 3 degrees, assessed by a numerical scale of pain intensity. The swelling and pathological barrier in the area of the inguinal fascia on the right hip were removed, to improve the stereotype of the abductiob on the right hip, to reduce the shortening of the piriformis and iliopsoas muscles by 1 degree. The client reduced 10 kg body weight from 97.3 to 87.3 kg. Conclusion: During the therapies with my client, we managed to achieve the goals we set before the intervention. The client is still motivated to follow the set recommendations.
