Abstract:

Chromozomové aberace jsou jednou z významných příčin neplodnosti, opakovaných potratů a narození dítěte svývojovými vadami. Molekulárně-cytogenetické metody, využívané v reprodukční medicíně umožňují detekovat početní i strukturní chromozomové abnormality. Podle získaných výsledků se léčba neplodnosti individuálně upravuje. Pacientům, jimž byly detekovány chromozomální abnormality vkaryotypu je doporučováno preimplantační genetické testování embryí, které vede ke zvýšení šance na otěhotnění a narození zdravého potomka. V rámci této práce byly analyzovány karyotypy v letech 2013–2020 v centru asistované reprodukce REPROMEDA. Celkem bylo hodnoceno 7505 pacientů metodou klasického karyotypování. Bylo zjištěno 1,36 % chromozomových aberací, které byly potvrzeny metodou fluorescenční in situ hybridizace. Až polovina těchto aberací, tvoří translokace. Dalšími častými aberacemi jsou inverze a aneuploidie pohlavních chromozomů. Dále bylo v rámci práce vyšetřeno v období 2015–2020 celkem 122 vzorků choriových klků potracených plodů metodou SNP array. Bylo detekováno 37 % chromozomálních abnormalit ve tkáni potracených plodů, z nichž největší část tvořily aneuploidie celých chromozomů.

Abstract:

Chromosomal aberrations are one of the major causes of infertility, recurrent miscarriages and the birth of a child with developmental defects. Molecular-cytogenetic methods used in reproductive medicine detect and describe these numerical and structural aberrations. According to the obtained results, the treatment of infertility is individually adjusted. Patients who have been detected with karyotype abnormalities are recommended for preimplantation genetic testing of embryos, which significantly increases the chances of getting pregnant and giving birth to a healthy offspring. As part of this thesis, karyotypes in the years 2013–2020 were analysed in the centre of assisted reproduction REPROMEDA. A total of 7505 patients were evaluated by the method of classical karyotyping. Chromosomal abnormalities were identified in 1.36 %, which were confirmed by fluorescence in situ hybridization. Up to half of these pathologies form translocations. Other common aberrations are inversion and aneuploidies. The work also includes a set of analyses of 122 chorionic villus samples of aborted foetuses, analysed by the SNP array method in the period 2015–2020. More than 37% of chromosomal abnormalities were detected in the tissue of aborted foetuses, the largest part of them were aneuploidies of whole chromosomes.
