Management screening investigation of hearing defect by newborn

Abstract:

Cíl: Cílem práce bylo zjištění organizace screeningového vyšetření ve vybraných zdravotnických zařízeních. Druhým cílem bylo zjistit, zda jsou rodiče dítěte edukováni o vyšetření sluchu u jejich novorozence. Metodika: Výzkumný soubor tvořily sestry na novorozeneckých odděleních a sestry na ušním, nosním, krčním oddělení. Výzkum byl realizován ve čtyřech nemocnicích Moravskoslezského kraje: nemocnice Třinec, Frýdek-Místek, Karviná a Fakultní nemocnice Ostrava. Technikou sběru dat byl nestandardizovaný dotazník vlastní konstrukce, který nám měl pomoci zjistit znalosti sester v oblasti otoakustických emisí a vyšetření s nimi spojeného. Výzkumný soubor tvořilo 72 všeobecných sester. Výsledky: Byly zjištěny rozdíly ve vědomostech o vyšetření. V Ostravě byl zjištěn správný postup při vyšetření. V Třinci a v Ostravě se vyšetřuje 3. ? 7. den po narození. Bylo zaznamenáno, že v Ostravě vyšetřují sestry neonatologického oddělení, v Třinci zajišťuje vyšetření ORL lékař a sestra. Vyšetřováni ve všech nemocnicích byli všichni novorozenci, nejen rizikoví. Rodiče nemají často přehled, že toto vyšetření u jeho dítěte proběhlo, bylo zkoumáno, že nedostávají edukační materiály. Dalším získaným údajem bylo, že v žádné nemocnici nemají centrální registr vad. Výsledky ukazují na rodiče, že se nedostavují na kontroly. Management screeningu probíhá v každé nemocnici různě, není stanoven celoplošně. Závěr: Pozdní diagnostika sluchové vady ovlivňuje celkový stav dítěte, jeho zařazení do běžného života a rozvoj celkové osobnosti. Z tohoto důvodu je podstatné klást důraz na edukaci rodičů v porodnici, dále zvýšit povědomí sester pracujících na neonatologii o důležitosti vyšetření. Přínosem této práce je ukázka jednoduchého vyšetření, které pomůže zajistit dítěti úspěšný start do života.

Abstract:

The target: The aim of the work was to find out an organization of a screening exam in chosen medical organizations. The other aim was to find out if parent of a newborn are educated about hearing exam of their child. Methodology: The research sample consisted of nurses in a neonatal care and the nurses at the ear, nose, throat department. The research was materialized in four hospital of Moravian-Silesian Region: hospital in Třinec, Frýdek ? Místek, Karviná and College hospital of Ostrava. The method of gathering the datas was a non- standardized questionnaire created by me out the knowlewdge of nurses in an otoacustic emission and the exams connected with them. The medical team contained 72 nurses. Results: The differences in the physical examinations were detected. In Ostrava, the process of the physical examination was right. In Ostrava and Třinec, the physical examination is done between the third and seventh day after the birth. It was registered that in Ostrava, the physical examination is done by nurses working in neonatology department. In Třinec, the physical examination is provided by an otorhinolaryngologist and a nurse. Not just those newborns who were in a risk, but all of them were examined. Parents often do not know, whether their baby was examined or not. They do not obtain any educational materials. Another found indication was that none of the hospital has central register of defects. The results show that parents do not take their baby for further medical checks. Management screening takes place in every hospital in different ways, not fully established. Conclusion: A late diagnosis of hearing defect influences the whole state oh child, their inclusion into the everyday life and development of the whole personality. Becouse of that is very important to inform parents in maternity hospital, also to increase to knowledge of nurses working in a neonatal care unit about the importance of this exam. The benefit of this work is to show how easy the exam is and ensure a successful child´s start of a new life.
