NRAS mutations in patients with acute myeloid leukemia

Anotácia:

Diplomová práce se věnuje studiu mutací v genu NRas u pacientů s de novo akutní myeloidní leukémií (AML). Mutace v genu NRas jsou u AML pacientů nejčastějšími mutacemi genů rodiny Ras. Mutace genu lze identifikovat u přibližně pětiny všech AML nemocných přičemž mutace frekventně postihují kodony G12, G13 (exon 2) a Q61 (exon 3). Prognostický význam mutací v genu NRas je zatím stále nejasný. Analyzovaný soubor tvořilo 258 konsekutivních AML pacientů. U 58/258 pacientů byly NGS analýzou (VAF ≥ 1,0 %) a následnou verifikační analýzou (Sangerovo sekvenování, hybridizace na stripech) identifikovány mutace v genu NRas. Mezi 58 NRas pozitivními vzorky bylo přítomno sedm pacientů s CBF+AML, čtyři z nich nesli aberaci inv(16) a tři pacienti t(8;21). Klonováním v bakteriálních vektorech byl u testovaného vzorku prokázán polyklonální charakter NRas mutací. U všech 13 testovaných pacientů s VAF ≥ 35 % byl analýzou remisních vzorků potvrzen somatický původ mutací.

Abstract:

This thesis focuses on mutations in the NRas gene in patients with de novo acute myeloid leukemia. Mutations in NRas gene are the most frequent Ras gene mutations in AML patients. These mutations can be identified in approximately one-fifth of all AML patients, with mutations frequently affecting codons G12, G13 (exon 2) and Q61 (exon 3). Prognostic significance of these mutations is still unclear. The analyzed sample contained 258 consecutive AML patients. Mutations in NRas gene were identified in 58/258 patients using NGS analysis (VAF ≥ 1,0 %) and subsequent verification analysis (Sanger sequencing and hybridization on strip).Among 58 NRas positive samples seven patients were diagnosed with CBF+AML, four of them carried aberration inv(16) and three patients t(8;21). By cloning, the polyclonal character of NRas mutations was verified in the tested sample. In all 13 tested patients with VAF ≥ 35 % analysis of remission, samples confirmed somatic origin of NRas mutations.
