Children`s eating habits and their relationship towards other lifestyle factors: The Implementation of the World Health Organization`s preventive programmes in the Czech Republic

Abstract:

Práce se snaží popsat a na základě výzkumu zhodnotit fungování preventivních programů na podporu zdraví v České republice (především program Health Promoting School – HPS) a pomocí experimentální implementace nového preventivního programu Světové zdravotnické organizace Nutrition-Friendly Schools Initiative (NFSI), který je více zaměřen na zdravou výživu a zdravý životní styl, posoudit význam školních preventivních programů pro formování výživových návyků a prevenci nadváhy a obezity dětí i širší školní komunity.V první (teoretické) části práce je analyzována struktura a koncepce dosavadních preventivních programů na podporu zdraví. Podrobně je představen nový preventivní program NFSI, jeho ideová východiska, požadavky a cíle a návaznost na předchozí preventivní programy WHO a ostatních partnerských institucí, které se na tvorbě mezinárodních školních programů podílejí. Pro tyto programy bylo dosud charakteristické široké, obecné zaměření na rozvoj zdravého školního prostředí. Program NFSI také vnímá nutnost komplexní podpory zdraví, zdůrazňuje však více své zaměření na podporu zdravé výživy a aktivního životního stylu, jakožto klíčových faktorů, které mají přímou souvislost s celosvětově narůstajícím výskytem obezity a neinfekčních onemocnění, které většinou s obezitou úzce souvisejí. Specifikem NFSI tedy je, že se snaží využít zkušeností se školními preventivními programy a propojit je s poznatky z implementace programů zaměřených na zdravou výživu.Ve druhé (analytické) části práce byla popsána a zhodnocena situace na vybraných základních školách v České republice v oblasti prevence a výchovy ke zdraví s ohledem na požadavky preventivního programu NFSI. Na základě 22 kritérií tohoto programu i s přihlédnutím k tématům programu HPS bylo analyzováno národní kurikulum, školní kurikula vybraných škol a jejich přístup k výchově ke zdraví. Byl sledován celkový přístup škol k preventivním programům na podporu zdraví a prezentace škol v této oblasti vůči veřejnosti. Byly také sledovány další charakteristiky, které mají vliv na výchovu ke zdraví, ať už to byly komunitní prvky v přístupu škol k výchově ke zdraví, materiální podpora zdravé výživy a zdravého životního stylu, tj. vybavenost škol pro zdravé stravování a pro výuku tělesné výchovy nebo celkový charakter školního prostředí. Pomocí semistrukturovaných rozhovorů s vedením zkoumaných škol, analýzy školních dokumentů a ostatních shromážděných informací byly identifikovány důvody, překážky a problémy, které školám případně brání požadavky intervenčních programů naplnit.Třetí (experimentální) část práce byla věnována zkušebnímu zavedení preventivního programu NFSI na třech vybraných základních školách s odlišnými charakteristikami. Implementace programu probíhala souběžně a jednotlivé kroky odpovídaly požadavkům manuálu NFSI.Výsledkem práce s konkrétním dopadem je skutečnost, že u všech tří škol se preventivní program NFSI podařilo úspěšně zavést. Během tohoto experimentu se ukázalo, že zavedení nového programu (především formální stránka implementace) je snazší pro školy s programem HPS nebo tam, kde má vedení školy se zaváděním preventivního programu předchozí zkušenost. Tyto školy zvládají bez problémů jak autoevaluace, tak tvorbu plánů a strategie.Při analýze situace před a po zavedení programu NSFI se zřetelně ukázaly některé podstatné přínosy, které program NFSI pro české školy má, resp. mohl by mít při celoplošném zavedení: 1. y a zformulují pro veřejnost svoje záměry a směřování v této oblasti. 2. Pořádáním akcí na podporu zdravé výživy a aktivního životního stylu nejen pro děti, ale i pro širší komunitu, školy posilují komunitní prvky spolupráce a vzájemné podpory mezi partnery v obci. 3. Vzděláváním všech pracovníků v oblasti výchovy ke zdraví a jejich účastí v kurzech s tematikou zdravé výživy a zdravého životního stylu školy napomáhají zkvalitnění výuky i tvorby zdravého školního prostředí. 4. Rozvinutou spoluprací s odborníky z různých oblastí zdravotnického sektoru by školy mohly snadněji zvládnout řešení případných zdravotních problémů dětí a zvýšily by úroveň prevence.Přínos programu je znatelný i pro školy zapojené do HPS, v oblasti nutriční výchovy totiž není tento program tak detailně propracován. Pro školu bez zkušeností s preventivním programem bylo zavádění NFSI sice náročnější, ale s o to viditelnějšími výsledky.Na základě výsledků výzkumu a s přihlédnutím k doporučením WHO formulovaných v preventivních programech je možno navrhnout cesty vedoucí k většímu zapojování škol do preventivních programů a k cesty vedoucí k lepší funkčnosti a účinnosti programů. Ze strany veřejných institucí by měla být rozvinuta systémová podpora (finanční, vzdělávací i organizační), podpora rozvoje komunitních prvků mezi školou a okolím a podpora spolupráce mezi pedagogickým a zdravotnickým sektorem včetně zkvalitňování vzdělání pedagogických pracovníků v oblasti prevence a zdravé výživy. Samy školy by měly usilovat o lepší spolupráci s okolím, podrobnější informovanost veřejnosti a kvalitnější prezentaci školy. Měly by se snažit o lepší využívání stávajících možností (vybavení školy, ochoty rodičů i zřizovatelů zapojit se do spolupráce), intenzívnější kooperaci s dalšími „stakeholdery“, o všestrannější podporu zdravé výživy a zdravého životního stylu, měly by podporovat důslednou snahu jednotlivců být ve výchově ke zdraví dětem pozitivními vzory a umožnit postupné doplňování vzdělání a kvalifikace školních zaměstnanců.

Abstract:

This paper attempts to describe and, on the basis of research, evaluate the preventive programmes for health promotion in the Czech Republic (in particular the programme Health Promoting Schools – HPS). Through the experimental implementation of a new preventive programme by the World Health Organization, the Nutrition Friendly School Initiative (NFSI), which is aimed more at healthy eating and a healthy lifestyle, this paper will also assess the importance of the school preventive programmes in forming nutritional habits and the prevention of weight problems and obesity amongst children as well as the wider school community.In the first (theoretical) section of the work, the structure and concept of the existing preventive programmes for health promotion are analyzed. The new preventive programme NFSI is introduced in detail with an examination of its overall concept, requirements and goals, as well as its connection to the previous preventive programmes of WHO and partner institutes who have shared in the creation of the international schools programme. Up until now these programmes have been characterized by a broad, general look at the development of a healthy school environment. The NFSI programme also takes into account the need for a comprehensive promotion of health; however, it places more emphasis on the promotion of healthy eating and an active lifestyle, which are key factors in the global rise in obesity and the non-infectious diseases with which it is closely associated. The NFSI’s specific purpose, therefore, is to use the experience of the schools preventive programmes and incorporate this into the information gained from the programmes directed at healthy eating.In the second (analytical) part of the work there was a description and evaluation of the situation in selected secondary schools in the Czech Republic in the areas of prevention and health education with regard to the requirements of the NFSI preventive programme. Taking into account the themes of the HPS programme and on the basis of the 22 criteria of the NFSI programme there was an analysis of the national curriculum, the school curricula of the selected schools and their attitudes towards health education. There was an examination of the overall attitude of the schools towards the preventive programmes for health promotion and the schools’ presentations to the public in this area. Other factors were also examined which influence health education, such as community elements in the school’s attitude towards health education, material support for healthy eating and a healthy lifestyle, i.e. how well the school is equipped for healthy eating and for teaching physical education as well as the overall character of the school environment. Through semi-structured conversations with the heads of the selected schools, analyses of school documents and other information, the reasons, obstacles and problems which might prevent the schools from fulfilling the requirements of the intervention programmes were identified.The third (experimental) part was dedicated to the trial NFSI preventive programme at three selected primary schools which differed in nature. The programme was introduced simultaneously and the individual steps met the requirements of the NFSI manual.One of the specific impacts from the results of this work is the fact that all three schools manged to successfully implement the NFSI preventive programme. During the trial it emerged that the implementation of the new programme (in particular the formal side of its implementation) was easier for schools with the HPS programme or where the heads of schools had previous experience with preventive programmes. With the creation of written strategies to improve the school environment and raise the standard of health education with an emphasis on healthy eating, schools will be able to better understand their priorities and formulate their goals and directions in this area for the public. 2. By staging events to promote healthy eating and a healthy lifestyle for both children and the wider community, schools strengthen community co-operation and mutual support between partners in the district. 3. The training of all staff in health education and their participation in courses with the themes of healthy eating and healthy lifestyles helps to improve the quality of education and creates a healthy school environment. 4. By encouraging co-operation with specialists from various areas of the health sector, schools could more easily attend to children’s health problems and the level of prevention would increase.The benefit of the programme is also evident for schools connected to HPS as its programme for nutritional education is not as detailed. For schools which had no previous experience with a preventive programme the implementation of NFSI was more demanding but it still had visible results.On the basis of the results and taking into account the WHO recommendations in the preventive programmes, it is possible to suggest routes which would lead to greater incorporation of schools into the preventive programmes, which in turn would lead to a more effective functioning of the programme. Public institutions could develop their systems for support (financial, educational and organizational) as well as support the development of community features between the school and its surroundings and promote co-operation between the educational and health sectors, including the improved education for teaching staff in the areas of prevention and healthy diets. The schools themselves should attempt to interact more with the local area, giving to the public more detailed information and a higher quality presentation of the school. They should try to make better use of what they have at their disposal (school equipment, the willingness of parents and authorities to work together), co-operate more intensively with other “stakeholders“, and promote more comprehensively healthy eating and lifestyles. They should support the concerted efforts of individuals to be positive role models for children in terms of health education and to facilitate the gradual further education of school staff.
