Support caring for the elderly in late stages of dementia

Abstract:

Cíl: Cílem diplomové práce bylo zjistit a popsat náročnost péče o seniory v těžké fázi demence. Porozumět subjektivním potížím a problémům pečujících, se kterými se potýkají při každodenním pečování. Dále pak najít vhodné formy podpory a pomoci, které by mohly přispět k co nejdelšímu setrvání těžce dementního seniora v domácím prostředí. Metodika: Pro kvalitativní šetření byla zvolena metoda interpretativní fenomenologické analýzy pomocí nestandardizovaného rozhovoru. Výzkumný vzorek tvořilo deset participantů, kteří pečují o nemocného s demencí, jehož nemoc postoupila do těžké fáze. Nemocná osoba má přiznaný minimálně 3. stupeň příspěvku na péči a žije ve společné domácnosti s participantem. Výsledky: Péče o těžce dementního nemocného je náročná nejen fyzicky, ale také psychicky. Vzhledem k nedostatku informací o samotné nemoci, jejím průběhu, o pečovatelských technikách, mají participanti strach a obavy, zda o svého blízkého nemocného dobře pečují. Rovněž vnímají absenci sociální podpory a pomoci. Cítí se být v pečování osamoceni, bez zájmu zdravotně sociálního systému o jejich blízké nemocné a také o ně samotné. Všichni by uvítali pravidelnou odbornou pomoc v podobě návštěv nějaké kompetentní osoby. Závěr: S rostoucím počtem nemocných demencí stoupá i počet rodinných příslušníků, kteří o pacienta bezprostředně pečují. Pečující se rozhodli neumístit svého příbuzného do zdravotnického zařízení a vzali na sebe zodpovědnost, že jejich příbuzný prožije kvalitní život, že mu bude poskytnuta potřebná péče, kterou bude vzhledem k nevyléčitelnosti nemoci potřebovat až do samotného závěru života. Již na počátku pečování potřebují pečovatelé dostatek informací o nemoci, možné léčbě, ale také o sociálně právní problematice. Vhodně zvolenými intervencemi lze zmírnit intenzitu zátěže a zlepšit zdravotní a psychický stav nemocného i pečovatele. Role rodinné péče je v systému dlouhodobé péče velmi významná. Sociální a zdravotnický systém ji příliš neoceňuje ani neulehčuje a tím zůstává především péčí z lásky.

Abstract:

The aim: The aim of the thesis was to find out and describe exacting character of care of seniors in a difficult phase of dementia. To understand personal difficulties and problems of caring people, which they face everyday. The next aim was to find suitable forms of support and help, which could contribute to extend time of a seriously demented senior´s staying at home. Methodology: For qualitative survey there was used a method of interpretative phenomenological analyzis using the non-standard dialogue. A research sample was composed of ten participants taking care of a demented person. The person was declared at least the third degree of a care contribution and lives in the same household as the participant. Results: The care of a seriously demented person is difficult not only physically but also mentally. Due to the lack of information about the illness itself, its progress and nursing methods, the participants are worried if they are able to take a good care of a sick person. They also feel deficiency in social support and help. They feel alone in caring, without an interest of social system about themselves and their sick relatives. All would welcome regular professional help in a form of a qualified stuff´s visit. Conclusion: With the increasing number of demented people, the number of caring relatives increases, too. They decided not to place their sick relatives into a medical facility, but take the responsibility to provide a quality life, necessary care and take charge of them, which will be, concerning the incurability of the disease, needed till the end of life. Caring people need enough information about the disease, possible treatment, but also about welfare system from the very beginning. It is possible to reduce load intensity and improve the state of health and psyche of both - a sick and a caring person by suitable chosen interventions. The family care role is very important in a long-term care system. But it is not very appreciated and eased by the social and health systems, so it mostly becomes the care out of love.
