Interoceptive awareness and hallucinations: Meditation induced changes in predictive processing of sensory information (a cross-sectional study)

Abstract:

Cílem této studie bylo přispět ke kritickému zhodnocení výzkumu meditační praxe zkoumáním některých neobvyklých a málo reportovaných jevů souvisejících s meditací a některých možných mechanismů jejich vzniku. Zejména jsem zde zaměřil pozornost na obecné smyslové halucinace a tělesné halucinace dříve spojované s meditací. Tyto jevy jsem zkoumal z pohledu paradigmatu prediktivního zpracování. Na základě předchozích studií jsou halucinace pravděpodobně způsobeny přílišným spoléháním na předchozí smyslové signály a malým spoléháním na aktuální smyslové signály. Také, na základě předchozích studií jsou meditace spojeny s větším spoléháním na předchozí smyslové signály vlastního těla (tj. interoceptivní sensibilita), ale ne nutně se spoléháním na aktuální smyslové signály vlastního těla (tj. interoceptivní přesnost). Z těchto předpokladů jsem předpokládal, že mechanismem vzniku halucinačních jevů souvisejících s meditací by bylo právě přílišné spoléhání na předchozí smyslové signály vlastního těla. Abych otestoval tento předpoklad, uskutečnil jsem svou vlastní průřezovou studii porovnávající meditující a nemeditující ohledně interoceptivní citlivosti a náchylnosti k halucinacím a korelaci těchto proměnných. Zohlednil jsem také různý rozsah meditačních zkušeností mezi meditujícími a abnormální tělesné jevy související s meditací. Interoceptivní sensibilita byla operacionalizována jako skór na sebehodnocení úlohy počítání srdečního tepu. Obecná halucinační náchylnost byla operacionalizována jako bodování na Cardiffské škále anomálního vnímání. Abych vyhodnotil abnormální tělesné jevy související s meditací, vytvořil jsem vlastní dotazník založený na předchozích zprávách o těchto jevech. Podle výsledků, a oproti předpokladům, interoceptivní sensibilita nesouvisela s výskytem žádných halucinačních jevů a ve vzorku meditujících byla dokonce okrajově mírně negativně korelována s obecnou náchylností k halucinaci. Meditátoři v průměru dosáhli vyššího skóre obecné náchylnosti k halucinacím, ale samotná zkušenost s meditací byla významně mírně pozitivně korelována pouze s tělesnými halucinacemi. Neexistovala žádná korelace meditační zkušenosti s obecnou náchylností k halucinaci. To bylo překvapivé vzhledem k tomu, že meditující v průměru dosáhli vyššího skóre interoceptivní senzibility, která vykázala (marginální) tendenci korelovat s obecnou náchylností k halucinacím ve vzorku meditujících. Původní rozdíl v obecném skóre náchylnosti k halucinaci byl pravděpodobně způsoben jiným faktorem než meditací, a dokonce mohl fungovat jako motivátor pro hledání duchovní praxe. Ukázalo se, že vztah proměnných je složitější, než se původně předpokládalo. Celkově lze usoudit, že meditace mění preciznost vážení tělesných reprezentací způsobem, který usnadňuje tělesné halucinace, nikoli však obecnou náchylnost k halucinaci, a nikoli prostřednictvím interoceptivní sensibility. Smíšená povaha výsledků a marginalita některých zjištění nicméně vyžadují další zkoumání a replikaci s větší velikostí vzorku, možná s implementací designu meditační intervence.

Abstract:

The aim of this study was to add to a critical evaluation of meditation practise, by investigating some of the unusual and underreported phenomena related to meditation, and some of the possible mechanisms of their coming about. Particularly, I focused my attention here on general sensory hallucinations and bodily hallucinations previously associated with meditation. I have investigated these phenomena from the perspective of the predictive processing paradigm here. Based on previous studies hallucinations are most likely caused by over-reliance on previous sensory signals and little reliance on current sensory signals. Also, based on previous studies, meditations are associated with greater reliance on previous own-body sensory signals (i.e., interoceptive sensitivity), but not necessarily with relying on current own-body sensory signals (i.e., interoceptive accuracy). Out of these assumptions I have hypothesised that the mechanism of coming about of the hallucinatory phenomena related to meditation would be precisely the over-reliance on the previous own-body sensory signals. To test this prediction, I have realised my own cross-sectional study comparing meditators and nonmeditators on the interoceptive sensibility and hallucination proneness and correlating these variables. I have also accounted for the different extent of meditation experience amongst meditators and the abnormal meditation-related bodily phenomena. The interoceptive sensibility was operationalised as scoring on self-report measures of heartbeat counting task, that relate to one’s subjective evaluation of his or her performance on the task. The general hallucination proneness was operationalised as scoring on a Cardiff Anomalous Perception Scale. To assess for the abnormal meditation-related bodily phenomena, I have created an authorial questionnaire based on previous reports of these phenomena. According to the results, and opposite to the predictions, interoceptive sensibility was not related to the occurrence of any hallucinatory phenomena, and in the meditators sample it was even marginally moderately negatively correlated with the general hallucination proneness. Meditators on average scored higher on the general hallucination proneness, but the meditation experience itself was significantly moderately positively correlated only with the bodily hallucinations. There was no correlation of the meditation experience with the general hallucination proneness. This was surprising due to the fact that meditators on average scored higher on the interoceptive sensibility score, which showed a (marginal) tendency to be correlated with the general hallucination proneness in their sample. The original difference in the general hallucination proneness score was then assumed to be caused by a different factor than the meditation, and that it even may have functioned as a motivator to the seeking out of spiritual practice. The relationship of the variables turned out to be more complex than originally assumed. In sum, it can be concluded that meditation changes the precision weighting of bodily representations in a way that it facilitates bodily hallucinations, but not the general hallucination proneness, and not via interoceptive sensibility. Nevertheless, the mixed nature of the results, and the marginality of some findings ask for further investigation and replication with larger sample size, perhaps with implementation of a meditation intervention design.
