Abstract:

Úvod. Termografické metody nacházejí využití i v oboru vaskulární medicíny, především při diagnostice pacientů s angiopatií, ale i tzv. syndromem diabetické nohy. Principem infračervené termografie (IRT) je bezkontaktní měření povrchové teploty, jejíž změna má spojitost právě s prokrvením jednotlivých částí lidského těla. Výhodou IRT je především neinvazivnost vyšetření a bezpečnost pro pacienta i vyšetřujícího. Obecným cílem disertační práce bylo experimentální ověření vhodné metodiky snímání povrchové teploty dolních končetin. Hlavním cílem předložené disertační práce bylo použití IRT v klinických měřeních a diagnostice, zejména u pacientů s problémy s prokrvením dolních končetin (DK). Práce shrnuje výsledky studia nejen vlivu revaskularizace na teplotní změny DK u vybraných pacientů, ale i abnormalitu povrchové teploty v případě rizikových pacientů a zdravé populace. Metody. Termografická měření byla provedena celkem u 212 osob, z nichž 74 tvořilo kontrolní skupinu. Hodnocena byla povrchová teplota DK se zaměřením na vzájemnou asymetrii teplot obou končetin. V rámci klinického měření byl sledován dlouhodobý vývoj povrchové teploty DK u pacienta s diabetem. 102 ambulantních pacientů bylo do studie zařazeno jako osoby dříve léčené, či s různým stupněm poškození v prokrvení, u nichž byla sledována hmatná pulzace na DK. Revaskularizační zákrok byl proveden u 29 pacientů, z nichž 21 podstoupilo revaskularizaci endovaskulárně (PTA) a 8 pacientů angiochirurgicky (bypass). Spolu s měřením povrchové teploty byla zjištěna i hodnota indexu kotník-paže (ABI), obě metody vždy před a po operačním zákroku. Výsledky. Průměrný rozdíl povrchové teploty DK v případě kontrolní skupiny nepřesahuje hodnotu 0,6 °C mezi končetinami. U pacientů měřených v rámci ambulantního vyšetření tento rozdíl činí 1,8 °C a v případě revaskularizovaných pacientů je rozdíl roven 1,4 °C před zákrokem. V celém vzorku revaskularizovaných pacientů medián změny teploty vzrostl o 0,4 °C (PTA) a o 2,2 °C (bypass) u revaskularizované končetiny. Medián změny hodnoty ABI pak vzrostl o 0,17 (PTA) a nárůst o 0,65 (bypass) u revaskularizované končetiny. Kontrolní končetina vykazuje pokles mediánu změny teploty o 0,5 °C (PTA) resp. o 0,2 °C (bypass) a hodnoty ABI o 0,03 resp. 0,05. Ve skupině ambulantních pacientů byl prokázán vztah přímé úměry mezi povrchovou teplotou DK a přítomnými periferními pulzacemi. Závěr. Výsledky ukazují, že IRT je užitečný nástroj pro sledování pacientů trpících angiopatií DK, který v souladu s dalšími diagnostickými technikami umožňuje hodnocení kvality prokrvení periferií pacienta na základě povrchové teploty, jež prokázalo i sledování pacientů s diabetem.

Abstract:

Background. There are several applications of infrared thermography (IRT) in the field of human medicine. This study is focused on vascular medicine, especially on the diagnosis of patients with angiopathy. The principle of IRT involves the detection of infrared radiation that can be directly correlated with the temperature distribution of a defined body region. IRT provides a non-invasive analysis tool with the advantage of safe patient examination. The overall aim of this study was to experimentally verify appropriate methods for surface temperature screening of the lower limbs (LL) using IRT. The main objective of the study was the use of IRT in clinical examination, especially in patients with blood circulation problems. This study evaluates the effect of revascularization on temperature changes of LL and summarizes the abnormalities in surface temperature observed in high-risk patients versus a healthy population. Methods. Thermographic measurements were performed in a total of 212 adults (74 served as a control). The surface temperature was evaluated with the main focus on thermal asymmetry in both limbs. In the patient with diabetes, a long term evolution of surface temperature of LL was studied. 102 outpatients were included in the study - part of them had been treated previously and the rest suffered from various stages of blood supply damage. These patients were diagnosed by the palpation of arterial pulse in the LL. Revascularization was performed in 29 patients. 21 of whom were revascularized by percutaneous transluminal angioplasty (PTA) and 8 patients were revascularized by bypass. Thermal images and the value of the ankle-brachial index (ABI) were obtained before and after surgery. Results. The average temperature difference between plantar feet did not exceeded 0.4 °C in the control group. In the group of outpatients, the mean temperature difference was 1.8 °C. In the group of revascularized patients, the temperature difference between feet was 1.4 °C before surgery. Median temperature change in the treated limb increased by 0.4 °C (PTA) and by 2.2 °C (bypass) and the median change of ABI in the treated limb increased by 0.17 (PTA) and by 0.65 (bypass) in the group of patients who underwent revascularization. The median change of temperature decreased by 0.5 °C (PTA) and by 0.2 °C (bypass) and the change of ABI values were 0.03 (PTA) and 0.05 (bypass) in the control legs. A direct relation between surface temperature and palpable pulse was observed in the group of outpatients. Conclusion. The observed results prove that IRT is a useful tool for monitoring patients with angiopathy of the lower limbs. Along with other diagnostic method, IRT allows quality evaluation of blood circulation based on surface temperature.
