Complex therapy of pelvic floor dysfunction

Abstract:

Cíl práce: dokázat účinnost komplexní fyzioterapeutické léčby při dysfunkci pánevního dna stresové močové inkontinence. Metodika: provedením kineziologického rozbor najít patologie hybného systému, které by mohly negativně ovlivňovat stav pánevního dna. Na základě vyšetřením per vaginam a zhodnocení stavu svalstva dle perfect schématu stanovit krátkodobý fyziotrapeutický plán. Fyzioterapeutická intervence probíhala tři měsíce s pacientkou se stresovou močovou inkontinencí 2. stupně. Pacientce bylo 68 let. Výsledky: byla zjištěna stresová močová inkontinence 2. stupně, špatná funkce svalsva pánevního dna ve všech polohách, zejména v sedě, hypertonus v oblasti krční páteře a svalstva bederní páteře, nedostatečná funkce hlubokého stabilizačního systému. Fyzioterapeutická intervence byla založena na manuálních mobilizačních technikách, stretchingu, Kegelovu cvičení pánevního dna, cvičení koceptu DNS, konceptu McKenzie a elektrostimulaci. V průběhu terapie bylo zaznamenáno zlepšení funkce pánevního dna a hlubokého stabilizačního systému, zmenšení hypertonu u hypertonického svalstva, vymizení bolestivosti krční páteře. Diskuze: porovnání dosažených výsledků se studiemi, které jsou součástí teoretické části této bakalářské práce. Závěr: Vlivem intenzivní fyzioterapeutické intervence trvající tři měsíce, bylo dosaženo zmírnění příznaků stresové inkontinece a zlepšení celkového stavu pacientky. Zlepšil se stav svalstva pánevního dna ve všech polohách, zejména v sedě, kde byla ze začátku terapie nulová svalová odpověď. Ačkoli nedošlo k úpnému odstranění inkontinence, můžeme hodnotit fyzioterapeutickou léčbu stresové močové inkontinence jako úspěšnou.

Abstract:

The aim of the thesis: prove the effectiveness of complex physiotherapy for pelvic floor dysfunction - stress urinary incontinence. Methodics: performing kinesiology analysis to find the pathology of the motor system, which could negatively affect the condition of the pelvic floor. On the basis of examination per vaginam and evaluating the condition of the muscles according to perfect scheme to determine short-term physiotherapy plan. Physiotherapy intervention lasted three months with woman with stress urinary incontinence 2nd degree. The patient was 68 years old. Results: stress urinary incontinence grade 2nd level was determined, insufficient function of the pelvic floor muscles in all positions, especially in a sitting position, hypertonus in the cervical spine and muscles in the lumbar spine, insufficient function of the deep stabilization system. Physiotherapy intervention was based on a manual mobilization techniques, stretching, Kegel's pelvic floor exercises, workout concept of DNS, the concept McKenzie and electrostimulation. an improvement of function of the pelvic floor and deep stabilization system, reducing hypertonus with hypertonic muscles, the disappearance of the pain of the cervical spine during the treatment. Discussion: comparison of the results with studies, which are described in teoretical part of this thesis. Conclusion: Physioterapy intervention provided improve the overall condition and reduction the symptoms of stress urinary incontinence of the patient. This intervention took three months. Improved condition of the pelvic floor muscles in all positions was found, especially in a sitting position. There were zero muscular responses at beginning of therapy. The physiotherapy treatmnet of stress urinary incontenence is evaluated as successful. Although incontinence have not disapear completely.
