Screening tests for humoral immunodeficiency

Abstract:

Včasná a správná diagnostika imunodeficitů je důležitá pro včasné zahájení léčby pacientů. Cílem této práce bylo zhodnotit vyšetření "vypočteného globulinu"(CG; celková bílkovina - albumin) jako screeningové metody pro průkaz hypogamaglobulinémií. U 67 pacientů s CVID, 30 pacientů s IgAD a skupiny 77 pacientů bez rozlišení diagnóz byla vyšetřena koncentrace IgG, celkové bílkoviny (CB) a albuminu (ALB). Naše výsledky ukázaly ve všech třech skupinách signifikantní korelaci IgG s CG. Více než 92 % pacientů s CG < 18 g/l mělo zároveň hladinu IgG pod referenčním rozmezím. Mezi 30 pacienty s IgAD nebyl žádný s CG < 18 g/l, pouze 4 pacienti měli nízkou hladinu IgG. Vyšetření "vypočteného globulinu" se jeví jako rychlá a finančně nenáročná metoda odhalující nízké hladiny IgG. Pro určení deficitů jiných imunoglobulinových tříd je ovšem tento test nedostatečný.

Abstract:

Early and appropriate assessment of diagnosis of immunodeficiency is very important to initiate a proper treatment on time. The aim of this bachelor thesis was to evaluate the "calculated globulin" (CG;total protein - albumin) test as a screening method for determination of hypogammaglobulinemia. The blood samples of the patients, who suffered from CVID, IgAD and samples of patients with unknown diagnosis were tested for IgG concentration levels of total protein (TB) and albumin (ALB).Our results show significant correlation of IgG and CG in all groups of patients. More than 92 % of the patients with CG < 18 g/l had levels of IgG below the reference range. In the group of 30 patients with IgAD, there was nobody with level of CG < 18 g/l, only 4 patients had low levels of IgG. The test of CG seems to be a cheap and quick method to determine suspicion on lower levels of IgG. However this test is no sufficient for determination of deficit of other immunoglobulin classes.
