Physiotherapy for patients with vigile coma.

Abstract:

Bakalářská práce je zaměřena na problematiku pacientů s vážným neurologickým deficitem, který vznikl následkem hypoxie mozku nebo kraniocerebrálního poranění. Tento deficit se obecně označuje jako vigilní kóma a je definován jako stav kompletní porucha vědomí. Pacienti v tomto stavu si neuvědomují sami sebe ani své okolí a prostředí, v němž se nacházejí. V teoretické části práce je popsáno vigilní kóma neboli apalický syndrom či vegetativní a problematika spojená s tímto tématem, zahrnující diagnostiku, klinický stav, prognózu nebo klinicky podobné stavy. Dále možnosti monitorace pacientů s takto vážným postižením a fyzioterapeutické postupy využívající se v terapii pacientů s výše zmíněnou diagnózou. Praktická část je zaměřena na sledování a terapii čtyřech konkrétních pacientů s danou diagnózou. Popisuje využití, v teoretické části popsaných, fyzioterapeutických postupů. Zvolená terapie využívala především prvků bazální stimulace, pasivního cvičení, polohování a stimulace dechu. Dále jsou zde popsány cíle a výsledky práce. Jedním z výsledků je potvrzení, že provedení kontaktního dýchání vede ke zlepšení aktivity a efektivity ventilace pacientů. Další poukazujena výbavnost úchopového reflexu u takto neurologicky závažně nemocných a na nejčastější příčiny vzniku těchto defektů centrální nervové soustavy, hlavní příčinou, jak se ukázalo, je kraniocerebrální trauma. Tyto výsledky jsou detailněji popsány v diskuzi práce a shrnuty v jejím závěru.

Abstract:

The bachelor thesis is focused on patients with severe neurological deficit caused by brain hypoxia or craniocerebral injury. This deficit is generally referred as a vigilant coma. It is defined as the state of a complete disturbance of consciousness. Patients in this state are unaware of themselves or their surroundings and environment. The theoretical part of the thesis describes a vigil coma or apalic syndrome and issues related to this topic. It includes diagnostics, clinical state, prognosis or clinically similar states, further options of monitoring patients with such severe disabilities and physiotherapeutic methods used in the therapy of patients with the above-mentioned diagnosis. The practical part is focused on monitoring and therapy of four particular patients with the given diagnosis. It describes the use of, in the theoretical part described, physiotherapeutic methods in the therapy of these patients. The chosen therapy used mainly features of basal stimulation, passive exercises, positioning and breath stimulation. It also describes goals and results of the work. That point to activity improvement and efficiency of ventilation of the patients after contact breathing. Further, it points to grip reflex in such seriously neurologically ill people and the most common causes of formation of these defects of central nervous system, the main cause, how it was shown, is the craniocerebral trauma. These results are described in the discussion of the work and summarised in its conclusion.
