Mouse models of myeloid leukemia

Abstract:

Akutní myeloidní leukémie (AML) představuje závažné maligní onemocnění krvetvorby s velmi špatnou prognózou. Jedná se o klinicky i molekulárně značně heterogenní onemocnění, které vzniká postupným hromaděním vícečetných genetických aberací. Sekvenování nové generace (NGS) umožnilo v posledních letech detailně analyzovat mutace u pacientů s AML a popsat klonální složení a vývoj tohoto onemocnění. Velký přínos v této oblasti ale představují i imunodeficientní myší modely využívané pro xenotransplantace primárních buněk pacientů, což umožňuje sledovat klonální změny v uměle vytvořených podmínkách. Cílem předkládané disertační práce bylo určit kritické parametry pro uchycení leukemických buněk v imunodeficientním myším modelu a pomocí mutační analýzy sledovat klonální složení a patogenezi AML pomocí xenotransplantačních studií. Primární vzorky pacientů AML byli xenotransplantovány do myších recipientů a výsledný engraftment byl studován pomocí průtokové cytometrie a analýzy 18 genů asociovaných s AML za použití NGS. Úspěšného engraftmentu (přihojení > 1 % lidských leukocytů v myší kostní dřeni) bylo dosaženo u 44 % vzorků. Jako nejvýznamnější parametry ovlivňující engraftment jsme identifikovali vysoké cytogenetické riziko a nedosáhnutí kompletní remise, zatímco mutační status měl na engraftment nižší vliv. Ukázali jsme, že kritické parametry pro přihojení buněk AML zahrnují komplexní interakce mezi mutacemi a ostatními AML charakteristikami. Na základě studia mutací v myším modelu a v době relapsu onemocnění u pacientů jsme definovali hierarchickou roli mutací DNMT3A, NPM1, FLT3 a NRAS. Dále jsme ukázali stabilitu hlavních klonů nesoucích pre-leukemické a transformační mutace a variabilitu subklonálních mutací, a to jak v myším modelu, tak v době relapsu onemocnění. Disertační práce přinesla nové poznatky o hierarchii nejčastějších mutací u AML a detailní informace ohledně rekapitulace AML v myším modelu.

Abstract:

Acute myeloid leukemia (AML) represents a malignant disease of the hematopoiesis with very poor prognosis. It is a clinically and molecularly heterogeneous disease caused by a gradual accumulation of multiple genetic aberrations. Recently, next generation sequencing (NGS) enabled to analyze mutations in AML patients and its clonal composition and development. Immunodeficient mouse models used for xenotransplantation of primary patient derived cells also provided a significant contribution in this area, allowing monitoring of clonal changes under artificially created conditions. The aim of the present thesis was to determine the critical parameters for engraftment of leukemic cells in the immunodeficient mouse model and to investigate the clonal composition and pathogenesis of AML using the xenotransplantation studies. Primary samples from AML patients were xenotransplanted into mouse recipients. The resulting engraftment was studied using flow cytometry and NGS analysis of 18 AML-associated genes. Successful engraftment (>1% of human leukocytes in mouse bone marrow) was achieved in 44% of samples. We identified high cytogenetic risk and failure to achieve complete remission as the most important parameters influencing engraftment, while mutation status had a lower influence on engraftment. We showed that critical parameters for AML engraftment involve complex interactions between mutations and other AML characteristics. Based on the study of mutations in the mouse model and at the time of relapse in patients, we defined the hierarchical role of mutations DNMT3A, NPM1, FLT3 and NRAS in AML. Furthermore, we demonstrated the stability of major clones carrying pre-leukemic and transforming mutations and the variability of subclonal mutations, both in the mouse model and at the time of relapse. The results have contributed to the clarification of AML recapitulation in the mouse model and hierarchy of the most common mutations in this disease.
