Long-term prognosis of patients hospitalized with acute heart failure

Abstract:

Cílem práce bylo vyhodnotit dlouhodobou prognózu pacientů propuštěných z hospitalizace pro akutní srdeční selhání, zjistit rozdíly mezi pacienty se zachovalou a sníženou ejekční frakcí levé komory, pomocí DRG kódování diagnóz odhadnout celkové náklady na léčbu chronického srdečního selhání v České republice. Dalšími cíli bylo zhodnocení vlivu různých dávek diuretik a hyperurikémie na dlouhodobou prognózu pacientů. V této práci bylo prokázáno, že do 2 let zemře a/nebo je pro srdeční selhání rehospitalizována polovina těchto pacientů, pouze třetina pacientů přežívá bez jakékoliv hospitalizace. Toto se odráží v nákladech na veškerou hospitalizační péči pacientů se srdečním selháním. Dávka kličkového diuretika koreluje se závažností srdečního selhání, ale neovlivňuje dlouhodobou mortalitu. Hyperurikémie je spojena s nepříznivým kardiovaskulárním rizikovým profilem těchto pacientů, ale léčba nízkou dávkou alopurinolu nezlepšuje jejich dlouhodobou prognózu.

Abstract:

The aim of the study was to evaluate the long-term prognosis of patients discharged from hospitalization for acute heart failure, to evaluate differences between patients with preserved and reduced left ventricular ejection fraction, to estimate the total expenditure for treatment of chronic heart failure in the Czech Republic. The other objectives were to evaluate the effect of different doses of diuretics and hyperuricemia on the long-term prognosis of heart failure patients. We found that within 2 years half of these patients died and/or were rehospitalized due to heart failure, and only one third of the patients survived without any hospitalization. This is associated with high overall costs of in-hospital care for patients with heart failure. The dose of a loop diuretic correlates with the severity of heart failure, but does not affect long-term mortality. Hyperuricemia is associated with an unfavorable cardiovascular risk profile of these patients but low-dose allopurinol treatment does not improve their long-term prognosis.
