Advencements on the Treatment of Acute and Chronic Skeletal Infections at the Traumatology Department

Abstract:

Úvod: V průmyslové oblasti Ostravska s rozvinutým hutnictvím a těžbou uhlí je historicky zaznamenáván vysoký počet těžkých úrazů. V posledních letech se zde také zvyšuje počet vysokoenergetických dopravních úrazů. Tato těžká poranění jsou i přes adekvátní léčbu zatížena množstvím komplikací, z nichž závažnou část představují infekce skeletu. Cílem této práce je zavedení a zhodnocení výsledků nejvhodnějších metod léčby poúrazových infekcí skeletu. Materiál a metodika: Retrospektivně zhodnocen soubor 65 pacientů léčených na septickém oddělení TC FN Ostrava v letech 2000-2006. Nejčastěji se osteomyeli-tida vyskytovala po otevřených zlomeninách. Všichni pacienti podstoupili kom-plexní léčbu osteomyelitidy.Výsledky léčby byly zhodnoceny po dvou letech od ukončené léčby. Diskuze: V léčbě osteomyelitidy došlo v posledních 3 dekádách ke zlepšení vý-sledků, protože se do rutinní léčby zavedly nové metody - distrakční osteogeneza dle Ilizarova, svalové laloky místní i volné, nová lokální antibiotika, biomateriály podporující kostní hojení. Nicméně některé problémy nejsou zcela vyřešeny, zvláš-tě dlouhá doba léčby. Výsledky: Za nejvhodnější klasifikaci vhodnou ke srovnávání souborů považuje-me klasifikaci Czierny-Mader. V patogeneze osteomyelitidy se nejčastěji uplatňuje infekce zlatým stafylokokem V léčbě osteomyelitidy je nejdůležitější chirugická fázovaná terapie doplněná léčbou antimikrobní a adjuvantní. Excelentního výsledku v léčbě osteomyelitidy tibie dle ASAMI skore bylo dosa-ženo u 39,3 % pacientů a dobrého výsledku u 39,3 % pacientů. V této souvislosti je třeba upozornit, že nejvíce léčených pacientů mělo IV. stupeň osteomyelitidy dle klasifikace Czierny-Mader. Závěr: Léčba osteomyelitidy zůstává i do budoucnosti výzvou pro traumatology a ortopedy, poněvadž je komplikovaná, ekonomicky náročná a svou délkou zatěžu-jící pacienta.

Abstract:

Introduction: The industrial Ostrava region, with a highly developed metallurgy and coal mining, has historically been associated with a high incidence of serious injuries. The number of high-energy injuries caused by traffic accidents has also been increasing in the recent years. These serious injuries are, despite the adequate treatment, associated with a large number of complications, most of which are manifested in the form of skeletal infections. The aim of this work is to introduce and evaluate the outcomes of the most suitable methods of treatment of the post-injury skeletal infections. Materials and Methods: We retrospectively evaluated a group of 65 patients treated at the septic department of the Traumatology Centre in Ostrava between 2000 and 2006. Osteomyelitis was most frequently observed in patients with open fractures. All patients underwent a complex treatment of osteomyelitis. The out-comes of the treatment were evaluated after two years following the completion of the treatment. Discussion: The outcomes of treatment of osteomyelitis have improved in the course of the last three decades; this fact was caused mainly by introduction of new treatment methods – Ilizarov distraction osteogenesis, local and free muscle flaps, new local antibiotics, biomaterials supporting bone healing. Nevertheless, some persisting problems have not been entirely solved, especially the long period of treatment. Results: We consider the Czierny-Mader classification to be the most suitable for the comparison of the patient groups. The most frequently observed pathogenesis of the osteomyelitis is the infection caused by Staphylococcus aureus. The most important in the treatment of osteomyelitis is a staged surgical therapy, supplemen-ted with antimicrobial and adjuvant treatment. We managed to obtain excellent results of the treatment of osteomyelitis of the tibia according to the ASAMI score in 39.3% of patients, satisfactory results were observed in 39.3% of patients. In that respect it is necessary to point out that majority of the patients was diagnosed with type - IV osteomyelitis according to Czierny-Mader classification. Conclusion: The treatment of osteomyelitis remains a challenge for trauma and orthopaedic surgeons, because it is complicated, economically demanding, and burdensome for the patient, especially due to its prolonged duration.
