Assessment of quality of life in the elderly with dementia in institutional care

Abstract:

Cíl: Cílem diplomové práce bylo zjistit kvalitu života seniorů s demencí v institucionální péči, pohledem pečujících osob. Dále pak zjistit souvislost mezi úrovní kognitivních funkcí, pohlavím, věkovými skupinami, stavem soběstačnosti, typem demence, vzděláním a četností návštěv. Metodika: Byla použita kvantitativní metodika pomocí standardizovaných dotazníků DEMQOL - zkrácená verze a dotazníku QUALID. Sběr dat probíhal v období květen-srpen 2023. Výzkumný soubor tvořilo 113 klientů ze 2 Domovů pro seniory s částí DZR. Profesionální pečující vyplňovaly dotazníky z pohledu svých pacientů. Výsledky: Při hodnocení kvality života u seniorů s demencí v institucionální péči bylo zjištěno, že statisticky významně ovlivňuje kvalitu života úroveň kognitivních funkcí a stupeň soběstačnosti. Pohlaví, věk, typ demence a četnost návštěv nemají na kvalitu života statisticky významný vliv. Závěr: Syndrom demence je jedním z nejčastěji se vyskytujících onemocnění ve stáří, což klade vysoké nároky na péči a její organizaci. Proto je nutné do hloubky porozumět samotnému onemocnění demence a jeho projevům. Vzhledem ke stárnutí obyvatelstva, prodlužování délky života, přítomnosti chronických a dlouhotrvajících nemocí, v posledních letech není cílem medicíny zdraví, ale kvalita života, její zlepšení nebo její co nejdelší zachování. Kvalitu života seniorů s demencí ovlivňuje mnoho důležitých faktorů, jako je např. bolest, psychická pohoda, deprese aj. Ne vždy jsou vnímány a vyhodnoceny správně.

Abstract:

Aim: The aim of the diploma thesis was to determine the quality of life of seniors with dementia in institutional care from the perspective of caregivers. Then to find an association between cognitive function, gender, age groups, self-sufficiency status, type of dementia, education, and frequency of visits. Methods: A quantitative methodology was used using the standardized questionnaire DEMQOL - abbreviated version and the standardized questionnaire QUALID. Data collection took place between May and August 2023. The research sample consisted of 113 clients from 2 a homemfor the elderly with a part of a home with a special regime. Professional caregivers filled out questionnaires from the perspective of their patients. Results: When evaluating the quality of life of seniors with dementia in institutional care, it was found that the level of cognitive functions has a statistically significant impact on the quality of life. Gender, age, type of dementia and frequency of visits do not have a statistically significant effect on quality of life. However, the degree of self-sufficiency has a statistically significant impact on the quality of life. Conclusion: Dementia syndrome is one of the most common diseases in old age, which places high demands on care and its organization, but it is also necessary to understand the disease itself and its manifestations in depth. Due to the aging of the population, the prolongation of life expectancy, the presence of chronic and long-lasting diseases, in recent years the goal of medicine is not health, but quality of life and the longest-term preservation of the quality of life or its improvement. The quality of life of seniors with dementia is influenced by many important factors, such as pain, psychological well-being, depression, etc. They are not always perceived and evaluated correctly.
