Risk factors for mild cognitive impairment

Abstract:

Populace celosvětově stárne, se stárnutím populace souvisí vyšší riziko demence. Demence postihuje asi 5 % populace ve věku 65 let. Mírná kognitivní porucha je přechodnou fází mezi kognitivními změnami u fyziologického stárnutí a časnou demencí. Amnestický typ mírné kognitivní poruchy je považován za předstupeň Alzheimerovy choroby. Riziko konverze je udáváno mezi 5 % až 20 % za rok. Podle současných studií je v populaci výskyt mírné kognitivní poruchy u osob starších 65 let přibližně 20 %. Cílem práce byla analýza vybraných rizikových faktorů mírné kognitivní poruchy. Teoretická část diplomové práce byla zaměřena na výskyt onemocnění, rizikové faktory a možnosti prevence. V praktické části pak byla zpracována data získaná z dotazníkového šetření v rámci projektu Genetika a epidemiologie mírné kognitivní poruchy. Respondenti byli vyšetřováni v ordinacích praktických lékařů v Ostravě a Olomouci, v nemocnici v Havířově, seniorských centrech v Českých Budějovicích a Českém Krumlově. Po podepsání informovaného souhlasu, byl s respondenty vyplněn dotazník zaměřující se na rizikové faktory mírné kognitivní poruchy a Montrealský kognitivní test. Podle skóre v Montrealském kognitivním testu, byli respondenti rozděleni do souborů případů a kontrol. Celkový počet respondentů byl 259 (123 případů a 136 kontrol). Z analýzy dat byly zjištěny statisticky významné rozdíly ve výskytu hypertenze (p = 0,019; OR = 1,81; 95 % IS = 1,096 - 2,99), periferní aterosklerózy (p = 0,0014; OR = 1,23; 95 % IS = 1,57 - 7,71) a zaměstnání s převahou fyzické zátěže (p = 0,007; OR = 2,04; 95 % IS = 1,21 - 3,42) mezi respondenty v souboru případů ve srovnání s kontrolními osobami. Naopak roli protektivního faktoru před vznikem mírné kognitivní poruchy by mohlo hrát zaměstnání s převahou duševní zátěže a společenské aktivity (například návštěvy divadel). Do budoucna se tato problematika jeví jako velmi významný zdravotnický problém veřejného zdravotnictví. Mírná kognitivní porucha by se měla dostávat do popředí zájmu výzkumu. Studie zabývajících se touto problematikou by mohly přinést nové poznatky a postupy v prevenci, diagnostice a léčbě jak u mírné kognitivní poruchy, tak i u její možné progrese do Alzheimerovy choroby, která je nejčastější příčinou demence.

Abstract:

Population is aging globally, aging population is associated with a higher risk of dementia. Dementia affects probably 5 % of the population aged 65 years. Mild cognitive impairment is the transient phase between cognitive changes in physiological aging and early dementia. The amnestic type of mild cognitive impairment is considered a precursor of Alzheimer's disease. Conversion risk is between 5 % and 20 % per year. According to current studies, the occurrence of mild cognitive impairment in people aged 65 years and older is approximately 20 %. The aim of this work was to analyze risk factors for mild cognitive impairment. The theoretical part was focused on the occurrence of disease, risk factors and possibilities of prevention. In the practical part, the data obtained through questionnaires in the framework of the project Genetics and epidemiology of mild cognitive impairment were processed. The respondents were examined in the surgeries of general practitioners in Ostrava and Olomouc, in the hospital in Havířov, senior centers in České Budějovice and Český Krumlov. After signing informed agreement, a questionnaire focused on risk factors for mild cognitive impairment and the Montreal Cognitive Assessment was completed with the respondents. According to score of the Montreal Cognitive Assessment, respondents were divided into cases and controls. Data analysis revealed statistically significant differences in the occurrence of hypertension (p = 0,019; OR = 1,81; 95% IS = 1,096 - 2,99), peripheral atherosclerosis (p = 0,0014; OR = 1,23; 95% IS = 1,57 - 7,71) and employment with a superiority of physical load (p = 0,007; OR = 2,04; 95% IS = 1,21 - 3,42) among the respondents in the cases and controls. On the contrary, the role of a protective factor before the onset of a mild cognitive disorder could play employment with a superiority of mental load and social activity. In the future, this issue appears to be a very important health problem in public health. A mild cognitive disorder should be the focus of research. Studies on this issue could bring new knowledge and procedures in the prevention, diagnosis and treatment of mild cognitive impairment as well as its possible progression to Alzheimer's disease, which is the most common cause of dementia.
