Abstract:

Mnohočetný myelom je druhou nejčastější diagnózou v oblasti hematoonkologie. I přes obrovské pokroky ve výzkumu, které pacientům přinesly roky života navíc, se většině pacientů dříve či později po absolvování první léčby onemocnění opět vrací. Mnohočetný myelom patří mezi genetická onemocnění. Pacient do léčby vstupuje s konkrétní genetickou výbavou, která určuje průběh onemocnění. Diplomová práce je zaměřena na vyhodnocení vztahů expresí genů na úrovni genových sad a mezi genovými sadami navzájem. Geny nefungují jako samostatné jednotky a společně mohou ovlivňovat daný znak nebo svým produktem regulovat expresi jiného genu. Změna vztahů mezi geny nebo celými genovými sadami se nemusí projevit zvýšenou či sníženou hladinou exprese genů. Může však i přesto vyvolat patologický proces. V práci bylo zjištěno, že mezi geny v rámci genových sad a mezi celými genovými sadami navzájem existují vztahy, které se liší u pacientů nově diagnostikovaných a pacientů, kteří prošli minimálně jednou linií léčby. Změny byly pozorovány u sad regulujících buněčný cyklus, buněčnou smrt a proliferaci buněk. Deregulace těchto genových sad může být jedním z faktorů, proč se onemocnění po absolvování první linie léčby opět vrací. V diplomové práci se dále zabývám napojením více mikročipových platforem pro každého pacienta. Tyto technologie hodnotí výskyt změn počtů kopií genů a genovou expresi. Výskyt změn počtů kopií genů představuje poměrně nově objevený druh variability, jehož nalezení záviselo na rozvoji čipů komparativní genomové hybridizace. Tyto čipy detekují změny po celém genomu s vysokým rozlišením. V práci bylo zjištěno, že změny množství DNA, zisky nebo ztráty, nemusí ovlivnit hladinu exprese genu. Ty změny, které vedou ke změně exprese, považujeme jako kauzální změny, které iniciují progresi onemocnění. U pacientů při diagnóze byl potvrzen vliv tumor supresorového genu TP53, jehož ztráta vždy vede k poklesu exprese genu. U pacientů v relapsu byl potvrzen vliv zisku onkogenu MYC, přestože zisk DNA je u poloviny pacientů kompenzován sníženou expresí genu. Ztráta genu TP53 a zisk genu MYC jsou asociovány s kratším časem do progrese.

Abstract:

Multiple myeloma is second most frequent hematologic malignancy. Despite the progress in development of new treatment therapies which prolonged patient survival, the disease returns after the first treatment line. Multiple myeloma is a genetic disease. The genetic profile drives the disease aggressiveness. The thesis is aimed at evaluation of co-expression of genes within and between genesets. Genes do not work separately. Genes cooperate to influence a given trait. Together, genes can regulate expression of other genes. The change in the relationship between genes does not have to be observed as a change in gene expression level. These changes still can cause the pathological process. In this thesis, it was established that there is a link between the genes within one gene-set and between gene-sets mutually that differ in newly diagnosed patients and patients who undergone at least one treatment line. The changes were observed in sets that regulate the cell cycle, cell death and cell proliferation. Deregulation of these gene-sets may be one of the factors why the disease after undergoing of first treatment line returns. Next aim of this thesis was to integrate data from microarrays which evaluate gene expression and copy number variation. The copy number variation is a new kind of variability. It was discovered thanks to the development of arrays based on comparative genomic hybridization. These arrays enable the detection of copy number changes on whole genome scale with high resolution. It was found that the changes on DNA level, gains or losses, do not have to influence the gene expression dosage. The changes that lead to the change in gene expression level are considered as key drives of progression. For newly diagnosed patients the influence of tumor suppressor gene TP53 was confirmed. Loss of this gene is associated with low expression for all of patients with the loss. In group of relapsed patients the influence of gain of oncogene MYC was confirmed despite the fact that the gain was compensated by decrease in gene expression dosage in half of the patients with the gain. These changes were associated with shorter time to progression.
