Il protocollo di riabilitazione per un paziente sportivo operato per lesione SLAP del cercine glenoideo della spalla: caso clinico

The rehabilitation protocol for a sports patient operated for a SLAP lesion of the glenoid labrum of the shoulder: clinical case

Abstract:

SLAP lesions (Superior Labral tear from Anterior to Posterior) are antero-superior margin lesions of the glenoid rim at the point where it is connected to the biceps tendon, affecting both the anterior and posterior portions. It is a typical injury of athletes caused mainly by trauma due to uncontrolled throwing gestures or violent falls on the limb. This injury very often occurs of such magnitude that arthroscopic surgery is necessary. The current rehabilitative therapy protocols provide for the possibility that the patient returns to sports no earlier than 4 months after the operation and that, for overhead sports, longer times may be required, at least 6 months after the intervention. Rehabilitation must be carefully planned in different phases, setting specific objectives for each of them, to be reached in regular periods of about 4/5 weeks. Only upon reaching the set goals, it is possible to move on to the next phase until the patient recovers completely from the injury and return to normal competitive activity. In this work, after describing the anatomical, biomechanical and physiological characteristics of the scapulohumeral joint, the types of SLAP lesions and one of the main protocols related to their treatment are treated. As a clinical case, we followed a young patient, M.F., who reported a SLAP lesion to the left upper limb, due to an injury suffered during a football match, and that has been required the need for arthroscopic surgery. The patient followed a course of rehabilitative therapy, with the application of different techniques and structured over a period of 18 weeks divided into 5 phases, which led him to total recovery for competitive activity.

Abstract:

La SLAP (Superior Labral tear from Anterior to Posterior o "lacerazione antero-posteriore del labbro glenoideo superiore") ? una lesione del margine antero-superiore del cercine glenoideo nel punto in cui esso ? collegato al tendine del bicipite, con interessamento sia della porzione anteriore che di quella posteriore. ? una lesione tipica degli atleti causata soprattutto da traumi dovuti a gesti incontrollati di lancio o da violente cadute sull'arto. Molto spesso questo infortunio si presenta di tale entit? da rendere necessario l'intervento chirurgico in artroscopia. Gli attuali protocolli di riabilitazione fisioterapica prevedono la possibilit? che il paziente ritorni a svolgere l'attivit? sportiva non prima dei 4 mesi successivi all'operazione e che per gli sport overhead possono essere richiesti tempi pi? lunghi, di almeno 6 mesi dall'intervento. La riabilitazione deve essere accuratamente programmata in diverse fasi ponendo per ciascuna di esse degli obiettivi ben precisi da raggiungere in periodi regolari di circa 4/5 settimane. Soltanto al raggiungimento degli obiettivi prefissati, si pu? passare alla fase successiva fino al recupero totale dall'infortunio e al ritorno alla normale attivit? agonistica. Nel presente lavoro, dopo aver descritto le caratteristiche anatomiche, biomeccaniche e fisiologiche dell'articolazione scapolo-omerale, vengono trattate le tipologie di lesioni SLAP ed uno dei principali protocolli relativi al loro trattamento. Come caso clinico, ? stato seguito un giovane paziente, M.F., il quale, a causa di un infortunio subito durante una partita di calcio, ha riportato una lesione SLAP all'arto superiore sinistro che ha richiesto la necessit? al ricorso ad intervento chirurgico in artroscopia. Il paziente ha seguito un percorso di riabilitazione fisioterapica, con l'applicazione di diverse tecniche e strutturato in un periodo di 18 settimane suddiviso in 5 fasi, che lo ha condotto al totale recupero per l'attivit? agonistica.
