Premature Birth and its Impact on a Woman's Psyche from a Midwife's Perspective

Abstract:

Tato bakalářská práce se zabývá Předčasným porodem a jeho vlivem na psychiku ženy z pohledu porodní asistentky. Je rozdělena na dvě části, a to teoretickou a praktickou. V teoretické části popisujeme problematiku předčasného porodu, příčiny a rizikové faktory, klinický obraz, diagnostiku, terapii a prevenci. Poté se zabýváme klasifikací předčasně narozeného novorozence a jeho zdravotními specifiky, které jsou s prematuritou spojené a systémem péče o něj v České republice. Třetí kapitola je věnována potřebám a jejich významu v lidském životě, hlavně specificky v období po předčasném porodu. Poslední kapitola se zabývá kompetencemi porodní asistentky v kontextu péče o ženu před, při a po předčasném porodu. Praktická část je z jedné strany věnována jednotlivým potřebám matek po předčasném porodu a zkušenostem v jejich naplňování, ze strany druhé se zabývá pohledem staničních sester neonatologických jednotek intenzivní a resuscitační péče na potřeby žen po předčasném porodu. Provedli jsme smíšený průzkum v rámci, něhož jsme zpracovali semistrukturované rozhovory se staničními sestrami neonatologických oddělení a dotazníkové šetření s matkami po předčasném porodu. Výstupem práce jsou doporučení pro praxi a zároveň může tato bakalářská práce sloužit, jako informační zdroj pro studenty ošetřovatelských oborů. Hlavním cílem této bakalářské práce je poukázat na problematiku psychosociálních potřeb žen po předčasném porodu a poskytnout pohled na naplňování potřeb ze strany respondentek po předčasném porodu a též staničních sester neonatologických oddělení, konkrétně neonatologických jednotek intenzivní a resuscitační péče.

Abstract:

This bachelor thesis focuses on Premature birth and its Impact on a Woman’s Psyche from a Midwife’s Perspective. It is divided into two parts, a theoretical and practical one. In the theoretical part, we are describing the problematics of premature birth, its causes and risk factors, the clinical picture, diagnostics, therapy and prevention. Then we are adressing the classification of a prematurely born infant and his health specifics, which are connected to prematurity and the system of care in the Czech republic. Third chapter we are dedicating to needs and their significance in human life, specifically in the period after preterm birth. Last chapter is looking into competencies of a midwife in the context of providing care to a woman before, during and after preterm birth. The practical part is from one point of the view dedicated to the individual needs of mothers after preterm birth and experiences with their satisfaction, while the second point of view focuses on the perception of station nurses of the neonatological intensive and resuscitative care units regarding women’s needs after preterm birth. We conducted a mixed methods research in which we processed semistructured interviews with the station nurses of the neonatological units and a questionnaire with mothers after preterm birth. The outcomes of this work provide reccomendations for practice and this work can simultaneously serve as an information source for students in healthcare programmes. The main goal of this bachelor‘s thesis to point out the problematice of psychosocial needs among women after preterm birth and to provide a perspective on satisfaction of these needs from the viewpoint of our respondents after preterm birth as well as the station nurses of the neonatological units, specifically the neonatological intensive and resuscitative care units.
