Specificities of Nursing Care for Patients with Secured Airways

Abstract:

Tématem této bakalářské práce jsou specifika ošetřovatelské péče u pacientů se zajištěnými dýchacími cestami. V teoretické části jsou uvedeny možnosti zajištění dýchacích cest v případě potřeby umělé plicní ventilace (UPV), její indikace a potenciální komplikace, které se mohou objevit u pacienta se zajištěnými dýchacími cestami. Dalším bodem teoretické části je detailnější popis specifik UPV v podkapitolách nastiňujících některé druhy umělé plicní ventilace, blíže se zabývá komplikacemi, které mohou při UPV vzniknout a zmiňuje i problematiku extubace. Stěžejním tématem teoretické části je ošetřovatelská péče o pacienta na UPV a její specifika, jako je monitorace, prevence ventilátorové pneumonie či komplexnost péče. Poslední kapitola se věnuje problematice stresorů, se kterými se může pacient se zajištěnými dýchacími cestami setkat v intenzivní péči, a které mohou ovlivňovat jeho psychické zdraví. Praktická část obsahuje vyhodnocení odpovědí získaných kvantitativní metodou prostřednictvním dotazníku, který vyplnilo 201 respondentů. Z přehledných grafů je možno vyčíst procento správných i nesprávných odpovědí respondentů v závislosti na řadě faktorů, například na dosaženém vzdělání, délce praxe či oddělení, na kterém respondent pracuje. Dotazník byl pomocí elektronického odkazu rozdistribuován do nemocnic v Brně na ta oddělení, která se vzhledem ke svému zaměření setkávají s pacienty se zajištěnýmí dýchacími cestami. Teoretické znalosti personálu v oblasti uvedené problematiky, jejichž úrověň zjištěná dotazníkovým šetřením je graficky zobrazena v praktické části této práce, jsou předpokladem pro výkon práce na takovém oddělení. Z výsledků práce vyplývá, že většina zdravotníků má dostatečné znalosti v oblasti ošetřovatelské péče o pacienta se zajištěnými dýchacími cestami, nicméně průběžná edukace je nedílnou součástí této specializované péče. Od personálu, který je edukován v oblasti svého působení lze očekávat profesionálnější přístup k řešení komplikací, které pacienty na UPV postihují. Cílem práce je teoretické shrnutí základních informací o ošetřovatelské péči u pacientů se zajištěnými dýchacími cestami a případné prohloubení znalostí čtenáře o této problematice.

Abstract:

This bachelor’s thesis focuses on the specifics of nursing care for intubated patients. The theoretical part enumerates existing tracheal intubation techniques in cases requiring mechanical ventilation of the lungs, indications for mechanical ventilation and potential complications occurring in intubated patients. A more detailed description of mechanical ventilation specifics is provided in subchapters outlining the most common types of mechanical ventilation while focusing primarily on possible mechanical ventilation-related complications, and touching upon the topic of extubation. The theory is approached from the viewpoint of nursing care for mechanically ventilated patients and its specific aspects such as patient monitoring, ventilator-associated pneumonia prevention and care complexity. The final chapter of the theoretical part deals with the issue of stressors pertinent to intubated patients in the intensive care and the negative impact such stressors may exhibit on patients’ psychological well-being. The empirical part of the thesis presents the results of a knowledge survey carried out among 201 respondents from the ranks of relevant medical personnel. Presented survey diagrams display the division of correct response percentages by a number of factors, such as respondent achieved education, length of work experience or medical department. The source survey had been distributed as a web page hyperlink sent to Brno hospital departments which, by the nature of their specialization, deal with intubated patients. Theoretical knowledge, whose level is presented in the empirical part of this thesis, is an essential prerequisite for working at these departments. The survey results demonstrate that a majority of medical personnel possess sufficient knowledge in the area of nursing care for intubated patients. Continuous education, however, remains an indispensable part of a successful job performance. Personnel receiving continuous education in their respective field are likely to take a more professional approach to solving treatment complications affecting patients on mechanical ventilation. This thesis aims to summarize essential information on nursing care for intubated patients and possibly deepen the reader’s knowledge of the topic.
