Experimental Justification of Increasing the Radical Approach towards Renal Tumor Treatment

Abstract:

Práce podrobně pojednává o stavu současných poznatků a o názorech na chirurgickou léčbu karcinomu ledviny a konfrontuje je s vlastními indikačními kritérii u poměrně rozsáhlého klinického souboru vlastních pacientů. Autor se přiklání k maximální možné radikalitě s preferenční snahou o záchovný resekční výkon na ledvině, kde je to technicky možné. V porovnání se stávajícími doporučenými standardními postupy jsou autorovy indikace nadhraniční. Otázka chování organismu při nezanedbatelné mase arteficielně alterovaného parenchymu ledviny následkem ischemizace hemostatickou suturou rozsáhlé resekční plochy je řešena vyhodnocením biochemických parametrů v experimentu na laboratorním zvířeti (miniprase). Při rozvahách oprávněnosti operační intervence u nádorů generalizovaných pokládá autor za hlavní indikační kritérium kvalitu života v rámci paliativního ovlivnění inkurabilního stavu. U nádorů s tumorózním trombem dolní duté žíly je prezentována předoperační inzerce kaválního filtru jakožto originální nadstandardní prvek v prevenci peroperační plicní embolie, která jinak může končit fatálně (mors in tabula).

Abstract:

The kidney carcinomas belong to malignancies essentially hard to address by means of other than surgical treatment. Regarding their radioresistance, new approaches and methods have been being looked for to improve the prognosis of patients suffering from this disease. Hormonal therapy actually proved to be ineffective; therapy by cytostatic agents is disputable. As a result of multiyear intensive theoretical and clinical research, only few types of oncological diseases can be admitted as impervious to all used combinations of cytostatic treatment (this applies in curative as well as in adjuvant and neoadjuvant indications). Even the contemporary findings in the field of immunomodulatory therapy, which originally appeared enormously auspicious, are far from proving the initial enthusiasm. Anyway, the prerequisite for the immunomodulation of a metastasis process is primary chirurgical removal of the tumor origum – the kidney suffering from tumor. Yet until recently, the explicit method of choice for kidney carcinoma was represented by radical nephrectomy. Forehanded resection methods (so-called “nephron-saving” operations also known as “conservative surgical treatment”) are still out of the common repertory at most urology departments in the Czech Republic. In contrast, even the opinions on how radical the adequate treatment should be are not unanimous, especially in the cases of progressive local tumor process. Frequently, the tumor is evaluated as inoperable, the patient being only indicated to symptomatic treatment. However, the aggressive therapeutic approach may be especially justified in the cases of the manifestation of progressive metastases. The objective of this thesis is to present my own experience, acquired in surgical treatment of patients suffering from tumor diseases of kidney, based on knowledge obtained from experiments on animals.
