Moral distress in intensive care units during the COVID-19 pandemic

Abstract:

The thesis deals with the issue of moral distress and it´s occurrence in health care workers in intensive care units. The hesis is devided into theoretical and practical parts. In the theoretical part, the definitions of moral distress, determinants of moral distress are described, followed by tool for the assessment of moral distress. The last chapter of the theoretical part describes moral distress in the COVID-19 pandemic and how to manage moral distress. The empirical part is based on quantitative research in the form of a standardized questionnaire. The aim of the study was to find out the declared prevalence of moral distres in intensive care students and to find out the declared level/measure of moral distres in intensive care students and the specific situations in which the respondents experienced moral distres. The results of our survey showed that the reported prevalence of moral distress did not differ between respondents of different study formats. We found higher levels of moral distress among respondents of higher age, with longer experience in health care, and among respondents who had previously considered leaving their jobs. Specific situations that cause moral distress in respondents include providing aggressive care at the request of family or when no one decides to terminate. Based on the findings, recommendations for practise were proposed for both health care professionals in clinical practise and health care professionals preparing for the profession that focus on the prevention and avoidance of moral distress.

Abstract:

Diplomová práca sa zaoberá problematikou morálneho distresu a jeho výskytu u zdravotníckych pracovníkoch na jednotkách intenzívnej starostlivosti. Práca je rozdelená na teoretickú a praktickú časť. V teoretickej časti sú popísané definície morálneho distresu, determinanty vzniku morálneho distresu, následne sú popísané nástroje na hodnotenie morálneho distresu. V poslednej kapitole teoretickej časti je popísaný morálny distres v období pandémie COVID-19 a ako zvládať morálny distres. Empirická časť je založená na kvantitatívnom výskume formou štandardizovaného dotazníka. Cieľom štúdie bolo zistiť deklarovaný výskyt morálneho distresu u študentov intenzívnej starostlivosti a zistiť deklarovanú úroveň/mieru morálneho distresu u študentov intenzívnej starostlivosti a konkrétne situácie, pri ktorých respondenti pociťovali morálny distres. Výsledky nášho prieskumu preukázali, že uvádzaný výskyt morálneho distresu sa nelíši u respondentov rôznej formy štúdia. Vyššiu úroveň morálneho distresu sme preukázali u respondentov vyššieho veku, s dlhšou praxou v zdravotníctve, a u respondentov, ktorí už v minulosti zvažovali opustenie pracovnej pozície. Medzi konkrétne situácie, ktoré u respondentov spôsobujú morálny distres, patrí poskytovanie agresívnej starostlivosti na žiadosť rodiny, alebo v prípade, že nikto nerozhodne o jej ukončení. Na základe zistených skutočností boli navrhnuté odporúčania pre prax, ktoré sú určené pre zdravotníckych pracovníkov v klinickej praxi a aj pre zdravotníkov pripravujúcich sa na povolanie, ktoré sa zameriavajú na prevenciu a predchádzanie vzniku morálneho distresu.
