Effectiveness of colorectal screening

Abstract:

Úvod: Epidemiologické trendy ukazují výrazný pokles incidence a mortality na kolorektální karcinom (incidence o více než 30 % a mortalita o téměř 45 %) za posledních dvacet let, což je pravděpodobně způsobeno zavedením screeningového programu. Národní program screeningu kolorektálního karcinomu byl zahájen v ČR roce 2000 a od ledna 2014 je cílová populace zvána k preventivním vyšetřením prostřednictvím adresného zvaní, kdy pojišťovna zasílá pozvánky těm, kteří se neúčastní pravidelných preventivních prohlídek. Cílem práce bylo provést srovnání a hodnocení efektivity screeningu kolorektálního karcinomu v ČR v roce 2022 ve srovnání s "kovidovým" rokem 2021. Metodika: Při zkoumání jsem vycházela z dostupných dat národního onkologického centra ÚZIS. Pro vyhodnocení efektivity screeningu byl použity parametry - pokrytí cílové populace, podíl jedinců s pozitivním výsledkem TOKS a počet provedených screeningových kolonoskopií. Pro zpracování dat byla použita základní popisná statistika s použitím tabulkových grafů v Excel. Výsledky: V roce 2022 se zvýšilo pokrytí screeningem cílové populace o 2 % oproti předchozímu roku. Proporcionální nárůst tohoto ukazatele byl zaznamenán ve všech věkových skupinách, pohlavích a regionech. V podílu pozitivních výsledků TOKS v letech 2020-2022 došlo k významnému zvýšení pozitivity TOKS ve srovnání s předchozími roky až na 10 %. Největší počet provedených screeningových kolonoskopií byl v roce 2022 (k nárůstu došlo o 27 %) oproti předchozím letům 2020-2021, kdy byl zaznamenán pokles hodnot v důsledku pandemie Covid-19. Nejvýraznější nárůst byl zaznamenán ve věkové skupině 50-54 let. Mezi muži a ženy se hodnoty zvýšily proporcionálně. Mezi kraji byl největší nárůst počtu kolonoskopií pozorován v hlavním městě Praze, Moravskoslezském kraji a Středočeském kraji. Závěr: Od zavedení národního programu screeningu kolorektálního karcinomu v ČR roce 2000 se zvýšil počet prováděných screeningových metod. Oproti roku 2021 došlo v roce 2022 k určitým změnám, a to především v tom, že došlo k proporcionálnímu nárůstu hodnot u všech sledovaných ukazatelů: pokrytí cílové populace, podílu jedinců s pozitivním výsledkem TOKS a počtu provedených screeningových kolonoskopií.

Abstract:

Introduction: Epidemiological trends show a significant decrease in the incidence and mortality of colorectal cancer (incidence by more than 30% and mortality by almost 45%) over the past twenty years, which is probably due to the introduction of the screening program. The national colorectal cancer screening program was launched in the Czech Republic in 2000, and since January 2014 the target population has been invited to preventive examinations by means of an invitation, when the insurance company sends invitations to those who do not participate in regular preventive examinations. The aim of the work was to perform a comparison and evaluation of the effectiveness of colorectal cancer screening in the Czech Republic in 2022 compared to the covid year 2021. Methods: In my research, I relied on available data from the National Cancer Center ÚSIS. To evaluate the effectiveness of the screening, parameters were used - coverage of the target population, the proportion of individuals with a positive TOKS result and the number of screening colonoscopies performed. Basic descriptive statistics were used for data processing. Results: In 2022, screening coverage of the target population increased by 2% compared to the previous year. A proportional increase of these indicators was recorded in all age groups, genders and regions. In the proportion of positive TOKS results from 2020 to 2022, there was a significant increase in TOKS positivity compared to previous years. The largest number of screening colonoscopies performed was in 2022 (an increase of 27%) compared to the previous years 2020-2021, when a decrease in values was recorded due to the covid- 19 pandemic. The most significant increase was recorded in the 50-54 age group. Among men and women, the values increased proportionally. Among the regions, the largest increase in the number of colonoscopies was observed in the capital city of Prague, the Moravian-Silesian Region and the Central Bohemian Region. Conclusion: Since the introduction of a national colorectal cancer screening program in the Czech Republic in 2000, the number of screening methods performed has increased. Compared to 2021, there were some changes in 2022, mainly in that there was an increase in the values of all monitored indicators: in the coverage of the target population, the proportion of individuals with a positive TOKS result and the number of screening colonoscopies performed.
