Physiotherapy in patients with abdominal hernia

Abstract:

ABSTRAKT Úvod: Kýla je vysunutí útrob štěrbinou z dutiny břišní. Orgán vytlačuje pobřišnici výchlipkou, která tak představuje kýlní vak. Řešení je převážně operativní. Po operaci je volena fyzioterapie zaměřující se především na péči o jizvu a na aktivaci hlubokého stabilizačního systému páteře. Vždy musí být přizpůsobena stavu pacienta. Cíl: Cílem bakalářské práce je přispět k řešení problematiky fyzioterapie u pacienta s břišní kýlou. Metodika: V této práci je zpracována kazuistika 36letého pacienta s pupeční kýlou. V období od 27. 11. 2023 do 29. 12. 2023 probíhaly dvakrát týdně terapie v soukromé ambulanci fyzioterapie v Žatci, kterých bylo celkem deset. Během první terapie byl proveden vstupní kineziologický rozbor a navržen krátkodobý fyzioterapeutický plán. V rámci terapií bylo nejvíce využívána dynamická neuromuskulární stabilizace a Inovované Klappovo lezení. V průběhu poslední terapie proběhlo výstupní vyšetření spolu se stanovením dlouhodobého plánu fyzioterapie. Výsledky: Po terapiích je pacient bez bolesti v oblasti břicha. Bederní páteř je bolestivá pouze při zvýšené námaze. Zlepšilo se držení těla, snížil se tonus v oblasti krční páteře, břicha, došlo k napřímení celé páteře a těžiště je méně vychýlené. Dále byly protaženy původně zkrácené struktury v oblasti kyčelních kloubů, které jsou nyní bez patologie. Také se u pacienta zlepšila dynamika rozvíjení páteře, rozsah pohybu v kyčelních kloubech a aktivace hlubokého stabilizačního systému páteře spolu s respirační amplitudou Závěr: Po zhodnocení výsledků je zřejmé, že použité techniky a metody, zvolené a obsažené v krátkodobém fyzioterapeutickém plánu, byly smysluplné a měly dobré výsledky a účinek.

Abstract:

SUMMARY Introduction: A hernia is the protrusion of the bowels through a slit from the abdominal cavity. The organ pushes out the peritoneum through a bulge, which thus represents a hernial sac. The solution is mainly operational. Rehabilitation focuses primarily on scar care and primarily on the deep stabilization system of the spine. It must always be adapted to the patient's condition. Aim: The aim of the bachelor thesis is to contribute to the solution of physiotherapy in a patient with abdominal hernia. Methods: In this work, a case report of a 36. year old patient with umbilical hernia, who was diagnosed in 2023, is processed. In the period from 27. 11. 2023 to 29. 12. 2023, therapies took place twice a week in a private physiotherapy clinic in Žatec, of which there were ten in total. During the first therapy, an initial kinesiological analysis was performed and a short-term physiotherapy plan was proposed. As the main methodologies, I used Dynamic Neuromuscular Stabilization and Innovative Klapp Climbing. During the last therapy, the final examination and comparison of the results took place. Results: After the therapies, the patient is free of pain in the abdomen. The lumbar spine is algic only during increased exertion. Posture has improved, tone in the cervical spine and abdomen has decreased, the entire spine has been straightened and the center of gravity is less deviated. There was a stretching of the originally shortened structures in the area of the hip joints, they are without pathology. The deep stabilization system of the spine has been activated. Furthermore, the patient's spinal development dynamics and range of motion in the hip joints improved. The amplitude of breathing has also improved. Conclusion: After evaluating the results, it is clear that the techniques and methods used, chosen and included in the short-term physiotherapy plan, were meaningful and had good results and effect.
