Clinical and comparative studies of implants PC.CP DHS and the treatment of trochanteric fractures

Abstract:

Disertační práce je zaměřena na tématiku ošetřování pertrochanterických zlomeniny. Jsou zde posuzovány výhody a nevýhody techniky ošetření PC.C.P (Percutaneos Compression Plate) ve srovnání s konvenčním implantátem DHS (dynamic Hip Screw). V klinické části studie je zahrnuto 70 případů ošetřených pro každý typ implantátu. V obou skupinách jsou hodnoceny charakteristiky pacientů, faktory vypovídající o vlastní operaci a pooperačním průběhu, stejně tak další osud pacienta. V experimentální části je hodnocena pomocí matematického modelování pevnost fixace pro oba implantáty v případě stabilní a nestabilní pertrochanterické zlomeniny. Pro tyto situace jsou stanoveny deformačně napěťové analýzy a k charakteristice jsou využity hodnoty vycházející z deformačního posunu, redukovaného napětí a přetvoření. Byla nalezena výrazně vyšší stabilita v případě implantátu PC.C.P, který lépe dokáže lépe stabilizovat zlomeninu, přenášet síly při zatížení a stejně tak minimalizuje riziko selhání implantátu. Při vyhodnocení obou souborů bylo zjištěno pro implantát PC.C.P. signifikantní zkrácení operačního času, redukce krevní ztát a současně i snížení nutnosti následné krevní transfůze. Taktéž bylo statisticky průkazné snížení mortality pacientů sledované do 2 let od úrazu a lepší návrat k samostatnosti pacienta hodnocený podle Katze. U implantátu PC.C.P. nebylo zaznamenáno selhání osteosyntézy. Na základě výsledků můžeme považovat ošetření pertrochanterických zlomenin implantátem PC.C.P jako bezpečnou a přínosnou metodu léčby s minimalizací rizik komplikací.

Abstract:

The dissertation is aimed at the problematic of pertrochanteric fractures treatment. This dissertation describes advantages and disadvantages of PC.C.P (Percutaneos Compression Plate) treatment technique compared with conventional DHS implant (dynamic Hip Screw). The clinical part of study includes 70 cases of treatment for each type of implant. The characteristics of patients are evaluated in both groups with factors analyzing the own operation and post-operative process with the subsequent quality of patient`s life. The experimental part comprises the mathematic modeling evaluating the strength of the fixation of the both implants for stable and unstable pertrochanteric fractures. In that case the stress-strain analysis is used and the characteristic is defined based on the calculated values of deformation shift, reduced tension and transformation. It was recognized the distinctly higher stability for PC.C.P. implant ensuring the better fracture stabilization, transmits powers in strain and as well risk minimizing of implant failure. The evaluation of the both groups shows the significant shortening of operating time, the reduction of blood loss with decreased need of subsequent blood transfusion for PC.C.P. implant. Also the decreased mortality of patients followed up 2 years after trauma was statistically significant with better recovery of patient assessed by Katze. No osteosynthesis failure was detected for the PC.C.P. implant. Based on the results we can consider the treatment of pertrochanteric fractures by PC.C.P implant to be safe and beneficial method of treatment with the minimizing of risk complications.
