Assesment of bone mineral density using dual energy X-ray absorptiometry in pediatric patients

Abstract:

Úvod: Práce je rozdělena na části, ve kterých je stručně zmíněna anatomie a metabolismus dětské kosti, problematika osteoporózy, příčiny a rizikové faktory u dětí. V další části je zmíněn vývoj osteodenzitometrických metod. Bakalářská práce je zaměřena především na osteodenzitometrickou metodu, která využívá rentgenovo záření o dvou různých energií. Poslední část je věnována vlastní studii. Cíl: Cílem práce je získat údaje o kostní denzitě u dětských pacientů a srovnání výsledků získaných ze dvou protokolů měření a určit odchylky, stanovení průřezu výsledků hustoty kostního minerálu ze zastoupení dětských pacientů europoidní rasy na pracovišti kostní denzitometrie Fakultní nemocnice v Ostravě. Materiál a metoda: Do retrospektivní studie bylo zahrnuto 61 dětí ve věku od 6 do 17 let, z toho 35 chlapců a 26 dívek, kteří v roce 2009 podstoupili měření hustoty kostního minerálu metodou dvouenergetické rentgenové absorpciometrie. Měření bylo provedeno na přístroji Hologic QDR 2000, pomocí dvou protokolů měření (infant spine a lumbar spine). Diskuse: Ani děti nejsou ušetřeny stavům, při nichž dochází ke zlomeninám z důvodu snížení hustoty kostního minerálu. Výskyt zlomenin, které mohou vzniknout z důvodu osteoporózy, je u dětí mnohdy podceňován. Bývají častou komplikací nějakého primárního onemocnění či jeho léčby. Velmi důležitá je včasná diagnostika úbytku hustoty kostního minerálu a správná interpretace výsledků měření. Důraz je kladen i na mezioborovou spolupráci. Vyšetření a analýzu měření provádí školený radiologický asistent. Hodnocení výsledků vyšetření provádí lékař. Využití metody dvouenergetické rentgenové absorpciometrie se v dětské populaci od roku 1987 výrazně zvýšilo. Výsledek vyšetření dvouenergetické rentgenové absorpciometrie je z pohledu radiologa číslo, které odpovídá určité diagnóze, což toto vyšetření odlišuje od všech ostatních zobrazovacích metod. Výsledky: Na základě zhodnocení uvedeného souboru dětských pacientů je sestaven průřez výsledků. Závěr: Radiolog i radiologický asistent musejí být seznámen s indikací k vyšetření, příčinami, které by mohly ovlivnit správnou interpretaci výsledků. Za vhodnou volbu standardizované techniky vyšetření zodpovídá radiologický asistent. Klíčová slova: Osteoporóza u dětí, BMD, DEXA, denzitometrie

Introduction: The wor is dividend into parts, which are briefly mentioned anatomy and betabolism of child bone problems and osteoporosis, causes and risk faktors in childern, The next part is mentioned on the development osteodenzitometric methods. Bachelor´s thesis is focused on tool to a method that uses X-rays of two different energies. The last section is devoted to thein study. Aim: The aim is to obtain data on bone density in pediatric patients and compared the results obtained from two measurement protocols and to identify deviations, cross-sectional results of the determination of bone mineral density of the representation of pediatric patients europoid race in the workplace bone densitometry Faculty hospital in Ostrava. Materials and methods: By retrospective study included 61 children aged 6 to 17 years, 35 boys and 26 girls, who in 2009 underwent a bone mineral density measurements using dual energy X-ray absorptiometry. Measurements were performed on the device HOLOGIC QDR 2000, using two measurement protocols (Infant Lumbar Spine and Spine). Discussion: Even children are not spared states, resulting in the fractures due to reduced bone mineral density. The incidence of fractures, which may reset from osteoporosis, is often underestimated in children. Tend to be a frequent complication of some underlying disease or its treatment. Very important is early diagnosis of bone mineral density loss and correct interpretation of measurement results. Emphasis is placed on interdisciplinary collaboration. Examination and analysis of measurements performed by trained radiographer. Evaluation results of the tests performed by a surgeon. The use of the dual energy X-ray absorptiometry in the pediatric population since 1987 has increased considerably. Test results dual energy X-ray absorptiometry is a radiologist in terms of the number that corresponds to a particular diagnosis, this test which distinguishes it from all other imaging methods. Results and conclusion: Based on the evaluation of pediatric patients that file is compiled the results section. The radiologist and radiographer assistant must be familiar with the indications for examination, the causes that could affect the correct interpretation of results.
