Nursing Process in Care of Patient with Diabetes Mellitus

Abstract:

Cílem této bakalářské práce je vytvořit ošetřovatelský proces v péči o pacienta s diabetem mellitem (DM) vycházejícího z ošetřovatelských diagnóz NANDA-I taxonomie II. Tato bakalářská práce je rozdělena na dvě části, a to na teoretickou část a praktickou část. První kapitola teoretické části práce se zabývá epidemiologickou situací, dále klasifikací onemocnění, klinickým obrazem, diagnostikou, terapií a následně komplikacemi, prevencí a strukturou dispenzarizace pacienta s DM. V druhé kapitole teoretické části je začleněna role sestry při ošetřování pacienta s DM a specifika, která ošetřovatelská péče s sebou přináší. Role sestry se zaměřuje na aplikaci inzulinu, péči o rány, první pomoc u hypoglykemického kómatu. Následně je popsána diabetická dieta a nutriční doporučení. Předposlední podkapitolou je edukace a poslední podkapitolou je měření glykémie a selfmonitoring. Empirická část práce se zabývá vytvořením názorného plánu ošetřovatelské péče vycházejícího z ošetřovatelských diagnóz NANDA - I taxonomie II, jež se vztahují ke konkrétním potřebám klienta z domova pro seniory s onemocněním DM II. typu. Dále následuje podrobné zpracování 7 ošetřovatelských diagnóz. Ke každé ošetřovatelské diagnóze byly vytvořeny cíle ošetřovatelské péče, ke kterým byly stanoveny vhodné intervence a následně probíhala realizace ošetřovatelské péče u pacienta. Na základě realizace péče u pacienta proběhlo zhodnocení ošetřovatelských cílů.

Abstract:

The aim of this bachelor thesis is to develop a nursing process in the care of a patient with diabetes mellitus (DM) based on the nursing diagnoses of the NANDA-I taxonomy II. This bachelor thesis is divided into two parts, namely the theoretical part and the practical part. The first chapter of the theoretical part of the thesis deals with the epidemiological situation, the classification of the disease, the clinical picture, diagnosis, therapy and then complications, prevention and the structure of dispensary care for DM patients. In the second chapter of the theoretical part, the role of the nurse in the care of the patient with DM and the specifics that nursing care entails are included. The role of the nurse focuses on insulin administration, wound care, first aid in hypoglycemic coma. Subsequently, the diabetic diet and nutritional recommendations are described. The penultimate subchapter is education and the last subchapter is glycaemic measurement and self-monitoring. The empirical part of the thesis deals with the development of an illustrative nursing care plan based on the NANDA - I taxonomy II nursing diagnoses that relate to the specific needs of a nursing home client with type II DM. This is followed by a detailed elaboration of the 7 nursing diagnoses. Nursing care goals were developed for each nursing diagnosis, to which appropriate interventions were determined, and then nursing care was implemented with the client. Based on the implementation of care with the patient, an assessment of the nursing goals was conducted.
