Detekce a identifikace patogenních mikromycet metodou PCR-RFLP a možné aplikace v klinické praxi

PCR-RFLP Assay for Detection and Species Identification of Fungal Pathogens and Its Possible Applications in Clinical Practice

Abstract:

Byla vyvinuta rychlá a citlivá PCR-RFLP metoda pro detekci a identifikaci patogenních mikromycet a možnost její aplikace byla testována na klinickém materiálu leukemických pacientů s febrilní neutropenií, chlopních pacientů s infekční endokarditidou a u pacienta s konstriktivní perikarditidou. Byly analyzovány houbové, ribosomální ITS2 sekvence 42 patogenních mikromycet a do konzervativních oblastí navrženy primery umožňující detekci těchto mikromycet v jedné PCR reakci. Pro druhovou a rodovou identifikaci patogenních mikromycet bylo využito analýzy polymorfismu délky amplifikačních produktů (APLP) a polymorfismu délky restrikčních fragmentů (RFLP). Specificita a reprodukovatelnost této metody byla testována na 316 kmenech 42 druhů mikromycet. Citlivost detekce prezentované metody byla kolem 3 buněk Candida albicans v 1 ml krve a výsledky detekce a identifikace mikromycet z klinického materiálu byly k dispozici do 8 hodin od odběru vzorku. Pomocí této metody byla houbová DNA detekována v 21 vzorcích krve a jednom vzorku biopsie plic leukemických pacientů s febrilní neutropeníí, na třech chlopních pacientů s infekční endokarditidou a v jednom vzorku tkáně pacienta s konstriktivní perikarditidou. Standardní hemokulivace byla úspěšná pouze v 6 případech. Všichni ostatní pacienti byly jak kultivačně, tak i PCR negativní. Kultivačně izolované kmeny se druhově shodovaly s těmi, které byly detekovány a určeny PCR-RFLP metodou. V souhrnu můžeme říci, že PCR-RFLP je jednoduchá, rychlá, citlivá a reprodukovatelná metoda. Je aplikovatelná pro identifikaci kultivačně izolovaných kmenů a pro detekci patogenních mikromycet v tělních tekutinách a tkáních postižených pacientů. Po nezbytné validaci, by prezentovaná metoda mohla být začleněna do rutinní mykologické praxe.

Abstract:

A rapid and sensitive PCR-based assay for detection and identification of fungal pathogens was designed and applicability of this method was investigated in a group of children with cancer and febrile neutropenia (FN), group of the patients with culture negative infection endocarditis (IE) and patient with pericardial constriction caused by Candida albicans. The ITS2 sequences and adjacent regions of 42 fungal pathogens were analyzed and primers for detection of all analyzed fungal species were designed. Amplification product length polymorphism (APLP) and restriction fragment length polymorphism (RFLP) generated genus- or species- specific patterns. The specificity of the method was tested on 316 strains of 42 fungal species. The sensitivity of the method was about 3 cells of Candida albicans per 1 ml of blood. The results were available within 8 hours after sample collection. The PCR assay detected fungal DNA in twenty-five clinical samples from 10 leukemic patients with FN, three patients with IE and one patient with constrictive pericarditis. Blood-cultures were positive in only 6 samples. Remaining patients were both culture- and PCR-negative. Culture-isolated strains matched completely those obtained by PCR-APLP-RFLP identification. In conclusion, developed PCR-RFLP assay is rapid, sensitive, reliable and easy-to-perform. This approach could be potentially applied for fungi identification of cultured organisms, and detection of fungi pathogens in body fluids or infected tissues. Once validated, this method could be easily adopted in a routine mycological practice.
