Abstract:

Diplomová práce se zabývá zjištěním otázek a reakcí u pacientů s hematologickou malignitou v paliativní péči, bez možnosti kurabilního řešení onemocnění. Základním cílem této práce bylo zjistit, jaké jsou nejčastější dotazy a reakce pacientů s hematologickou malignitou na konci života. Teoretická část, jenž představuje východiska pro část empirickou, představuje základní fakta paliativní péče a nejčastější hematologické malignity, včetně jejich specifických vlastností a odlišností od ostatních solidních tumorů. Empirická část posléze prezentuje metodiku a výsledky kvalitativního průzkumu, který byl realizován prostřednictvím ad hoc výzkumného desingu. Ke sběru dat byly použity polostrukturované rozhovory se třemi skupinami respondentů (pacienti, nejbližší osoby pacientů a pečující zdravotnický personál). Výsledky průzkumné části diplomové práce ukazují, že nejčastější otázky pacientů s hematologickou malignitou na konci života, jsou spojené s obavou o blízkou budocnost. Respondenti dále uvádí neuvědomnění si skutečnosti v prvotní fázi sdělení závažné diagnózy a negaci stavu. Specifickým znakem těchto pacientů na konci života je nutná péče o ně ve zdravotnickém zařízení, na místo péče v domácí prostředí. Tento fakt výplývá z vyskytujicích se symptomů typických pro terminální stádium hematologické malignity.

Abstract:

The diploma thesis deals with the identification of questions and reactions in patients with haematological malignancy in palliative care, without the possibility of curable treatment of the disease. The basic aim of this work was to find out what are the most common questions and reactions of patients with haematological malignancy at the end of life. The theoretical part, which presents the basis for the empirical part, presents the basic facts of palliative care and the most common haematological malignancies, including their specific properties and differences from other solid tumors. The empirical part then presents the methodology and results of qualitative research, which was carried out through ad hoc research design. Semi-structured interviews with three groups of respondents (patients, patients and carers) were used to collect data. The results of the exploratory part of the diploma thesis show that the most common questions of patients with haematological malignancy at the end of life are associated with the fear of the near future. Respondents also report unaware of the facts in the initial phase of communicating a serious diagnosis and state negation. A specific feature of these patients at the end of life is the need to care for them in a medical facility, to a place of care in a home environment. This fact results from the occurring symptoms typical of the terminal stage of haematological malignancy.
