Food allergy vs. food intolerance - differences and diagnosis in dogs

Abstract:

Tato bakalářská práce se zabývá problematikou potravinových alergií a intolerancí u psů s důrazem na vysvětlení mechanismu jejich vzniku, klinických symptomů, diagnostických metod, následného terapeutického řešení a uvedení některých nejčastějších problematických složek krmiva. Přestože se alergie i intolerance na krmivo mohou projevovat podobně a často dochází k jejich záměně, zásadně se liší mechanismem svého vzniku. Potravinová alergie je imunologicky zprostředkovanou hypersenzitivní reakcí, která není způsobená pouze protilátkami IgE, ale často i buněčně zprostředkovanou hypersenzitivitou. Významný podíl na vzniku alergií má imunitní systém psa, konkrétně gastrointestinální imunita a orální tolerance. Naopak intolerance na potraviny je reakce neimunologická, vycházející z metabolických, toxických, farmakologických či idiosynkratických mechanismů. Mezi klinické projevy patří postižení především kůže a gastrointestinálního traktu. Pro potravinové alergie jsou typické kožní příznaky, zatímco gastrointestinální symptomy se spíše projevují u intolerancí. U nežádoucích reakcí je podstatná diagnostika. Nejspolehlivější metodou je eliminační dieta s následným provokačním testem. Serologické testy jsou ve veterinární praxi často využívané pro diagnostiku, nicméně spolu s kožními testy vykazují nízkou spolehlivost a měly by být využívány spíše jako doplňkové metody při výběru složek krmiva. Terapie spočívá ve vyhýbání se identifikovanému alergenu nebo problematické složce krmiva vyvolávající potíže. Těmito složkami jsou nejčastěji lepek, laktóza, kuřecí maso a vejce. Farmakoterapie se využívá ke zmírnění vzniklých akutních příznaků a nenahrazuje dietní management. Rozlišení alergie na krmivo a potravní intolerance bývá ve veterinární praxi obtížné, jelikož oba stavy mají podobné projevy i odezvu na terapii a často bývají vyvolány stejnými složkami krmiva. Správné rozlišení stavů je důležité pro stanovení diagnózy, volbu vhodného dietního režimu a následnou léčbu.

Abstract:

This bachelor’s thesis focuses on the issue of food allergies and intolerances in dog, with an emphasis on explaining the mechanisms underlying their development, clinical symptoms, diagnostic methods, subsequent therapeutic approaches and identifying some of the most common problematic ingredients in dog food. Although food allergies and intolerances may present similarly and are often confused they differ fundamentally in their mechanisms of development. A food allergy is an immunologically mediated hypersensitivity reaction that is not caused solely by IgE antibodies but often also by cell-mediated hypersensitivity. The dog’s immune system plays a significant role in the development of allergies, specifically gastrointestinal immunity and oral tolerance. In contrast, food intolerance is a non-immunological reaction based on metabolic, toxic, pharmacological or idiosyncratic mechanisms. Clinical manifestations primarily involve the skin and gastrointestinal tract. Skin symptoms are typical of food allergies, while gastrointestinal symptoms are more commonly seen in intolerances. Diagnosis is essential for adverse reactions. The most reliable method is an elimination diet followed by a challenge test. Serological tests are frequently used for diagnosis in veterinary practice; however, along with skin tests, they show low reliability and should be used primarily as supplementary methods when selecting feed ingredients. Therapy consists of avoiding the identified allergen or problematic feed ingredient causing the symptoms. These ingredients are most commonly gluten, lactose, chicken, and eggs. Pharmacotherapy is used to alleviate acute symptoms and does not replace dietary management. Distinguishing between food allergy and food intolerance is often difficult in veterinary practise, as both conditions have similiar manifestations and response to therapy and are often triggered by the same food ingredients. Correctly distinguishing betweeen these conditions is important for establishing a diagnosis, selecting an appropriate dietary regimen and subsequent treatment.
