Time trend analysis of prostate cancer and related explanatory factors

Abstract:

Malignant neoplasm of the prostate is the most common type of a cancer in the Czech Republic. The aim of this bachelor thesis is to evaluate time trend of the malignant neoplasm of prostate with regards to related explanatory factors, which are demographic aging of the population and introduction of prostate specific antigen into clinical practice. The theoretical part of this bachelor thesis deals with a brief description of the prostate from the point of view which considers prostate as a sex gland, a risk factors for malignant neoplasm of a prostate, its clinical stages and lastly PSA test is mentioned. Furthermore, theoretical part of thesis speaks about an epidemiological description of malignant neoplasm of prostate in the Czech Republic. International comparison of given neoplasm in Europe is also carried out. At the end I present an overwiev of statistical methods which are frequently used in trend evaluation. In the practical part, joinpoint regression was applied on data from National Cancer Registry. Using the given regression, the development of the incidence and mortality of malignant neoplasm of prostate in the Czech Republic was evaluated. It has been shown, that in the case of age-standardized values (world standard population), the incidence increases until the year 2010. After this year, all values are stabilized. Given trend corresponds with demographic aging of the population and also with the development of prostate specific antigen tests, in which there is a decrease in growth after 2010. In the case of age-standardized rate (world standard population) mortality, values decreasing yearly by 1,1% after the year 1998. This is associated with the successful capture of earlier clinical stages of prostate malignancy and improving treatment techniques. Also, because of international comparison joinpoint regression was applied to 20 European countries. At the end, countries were divided into groups based on the average annual percentage change in incidence and mortality over the last 10 years. These countries were also evaluated using the Human Development Index and Gross Domestic Product. The Czech Republic is in the largest group with increasing incidence rates and decreasing mortality rates.

Abstract:

Zhubné novotvary prostaty sú najčastejším onkologickým ochorením u mužov v Českej republike. Cieľom tejto bakalárskej práce je zhodnotenie časového trendu zhubného novotvaru prostaty s uvažovanými súvisiacimi vysvetľujúcimi faktormi, ktorými sú demografické starnutie populácie a zavedenie prostaticky špecifického antigén testu do klinickej praxe. V teoretickej časti tejto bakalárskej práce sa venujeme stručnému popisu prostaty ako pohlavnej žľazy, rizikovým faktorom ochorenia zhubného novotvaru prostaty, jeho klinickým štádiám a testovaniu prostaticky špecifického antigénu. Ďalej bol v teoretickej časti vykonaný epidemiologický popis zhubného novotvaru prostaty v Českej republike a medzinárodné porovnanie daného novotvaru v Európe. Následne na konci teoretickej časti je prehľad štatistických metód často využívaných pri hodnotení trendu. V praktickej časti bola joinpoint regresia aplikovaná na dáta Národného onkologického registra. Pomocou danej regresie bol zhodnotený vývoj incidencie a mortality zhubného novotvaru prostaty v Českej republike. Bolo preukázané, že v prípade vekovej štandardizácie na svetový štandard incidencia narastá do roku 2010, následne sú hodnoty stabilizované. Daný trend korešponduje ako s demografickým starnutím populácie, tak aj s vývojom počtu prostaticky špecifických antigén testov, u ktorého po roku 2010 dochádza k poklesu v náraste. U vekovo štandardizovanej mortality na svetový štandard hodnoty po roku 1998 začínajú klesať o 1,1 % ročne, čo je späté so záchytom skorších klinických štádií zhubného novotvaru prostaty a so zlepšujúcimi sa liečebnými technikami. Taktiež bola z dôvodu medzinárodného porovnania joinpoint regresia aplikovaná na 20 európskych štátov. Na konci boli jednotlivé krajiny rozdelené do niekoľkých skupín v závislosti na hodnotách priemernej ročnej percentuálnej zmeny incidencie a mortality za posledných 10 rokoch. Krajiny boli taktiež zhodnotené pomocou indexu ľudského rozvoja a hrubého domáceho produktu. Česká republika sa nachádza v najpočetnejšie skupine s rastúcimi hodnotami incidencie a klesajúcimi hodnotami mortality.
