Pre-hospital Emergency and Intensive Care for Patients with Acute Liver Failure

Abstract:

RŮŽIČKA, Denis. Přednemocniční neodkladná péče a intenzivní péče o pacienta s akutním jaterním selháním. Vysoká škola zdravotnická, o.p.s. Stupeň klasifikace: Bakalář (Bc.). Vedoucí práce: PhDr. Dušan Sysel, Ph.D., MPH, Praha 2025. Tématem bakalářské práce je přednemocniční neodkladná a intenzivní péče o pacienta s akutním jaterním selháním. Práce je složena z teoretické a praktické části. Teoretická část je zaměřena na seznámení čtenáře s problematikou akutního jaterního selhání, jeho diagnostikou, klasifikací, klinickým obrazem, etiologiemi s důrazem na intoxikaci MDMA a s komplikacemi spojenými s tímto stavem. Dále jsou v teoretické části popsána specifika přednemocniční neodkladné a intenzivní péče a teoretická východiska ošetřovatelského procesu. V praktické části je popsána přednemocniční a ošetřovatelská péče o pacienta s akutním jaterním selháním v důsledku intoxikace MDMA, která je realizována během převozu zdravotnickou záchrannou službou a následně během hospitalizace na KARIP. Popis péče na KARIP je popsán formou ošetřovatelského procesu. Na základě anamnézy pacienta jsou stanoveny ošetřovatelské diagnózy dle NANDA. U tří vybraných ošetřovatelských diagnóz jsou podrobněji rozebrány cíle ošetřovatelské péče, navržené intervence, jejich realizace a zhodnocení. V rámci práce byly navrhnuty doporučení pro praxi.

Supervisor: PhDr. Dušan Sysel, Ph.D., MPH, Prague 2025. The topic of the bachelor thesis is pre-hospital emergency and intensive care of a patient with acute liver failure. The thesis consists of theoretical and practical parts. The theoretical part is aimed at familiarizing the reader with the problems of acute liver failure, its diagnosis, classification, clinical picture, etiologies with emphasis on MDMA intoxication and complications associated with this condition. Furthermore, the theoretical part describes the specifics of pre-hospital emergency and intensive care and the theoretical background of the nursing process. The practical part describes the pre-hospital and nursing care of a patient with acute liver failure due to MDMA intoxication, which is implemented during transport by the ambulance service and subsequently during hospitalization at the Karolinska Regional Hospital. The description of care at KARIP is described in the form of a nursing process. Based on the patient's history, nursing diagnoses are made according to NANDA. For three selected nursing diagnoses, the goals of nursing care, proposed interventions, their implementation and evaluation are discussed in detail. Recommendations for practice have been proposed.
