Influence of horse´s movement to the rider´s musculoskeletal system

Abstract:

Cíl: Cílem mé práce je přispět k problematice fyzioterapie ve sportovním jezdectví. Metodika: Bakalářskou práci jsme zpracovala jako případovou studii. Tedy kazuistiku 30 leté sportovní jezdkyně, u které byly vyšetřeny funkční poruchy pohybového aparátu, v důsledku nadměrné zátěže. K vypracování kazuistiky jsem použila metody pozorování, měření a testování. K odebrání anamnézy a dalších důležitých informací o pacientčině stavu, jsem využila metodu rozhovoru, která vychází z mluvené komunikace. Jako další metodu jsme si vybrala metodu pozorování, kterou jsem využila k vyšetření aspekcí a pro objektivní zhodnocení průběhu jednotlivých terapií. Metodu měření jsem použila k zjištění konkrétních hodnot a využila jsme k tomu goniometr a krejčovský metr. Na základě těchto informací jsem provedla další speciální testy a z výsledků všech vyšetření jsem navrhla krátkodobý fyzioterapeutický plán. Plán byl realizován během 10 návštěv, po dobu 8 týdnů. V terapii jsem použila tyto metody a koncepty - měkké a mobilizační techniky, postizometrickou relaxaci, reciproční inhibici, strečink, kinesiotaping, prvky DNS (Dynamické neuromuskulární stabilizace), inovované Klappovo lezení, senzomotoriku a Centered Riding. Pacientka byla během terapií instruována k domácí autoterapii a k zapojení prvků terapie (trupová stabilizace) během jízdy na koni. V polovině bloku terapií jsme provedla kontrolní vyšetření zaměřené na hlavní patologie. Na závěr absolvovala pacientka výstupní kineziologické vyšetření a dle výsledků byl stanoven dlouhodobý fyzioterapeutický plán. Výsledky: V průběhu terapie došlo k odstranění bolesti,prostřednictvím uvolnění kloubních blokád (SI skloubení) a tím vymizení reflexních změn,charakteru taunt band a tender point (m.piriformis, m. iliopsoas - vlevo, m.levator scapulae, m. trapezius - bilaterálně). Došlo i k protažení zkrácených struktur - adduktorů a flexorů kyčelního kloubu. Zlepšil se dechový stereotyp a zapojení hlubokého stabilizačního systému. Dále došlo k výraznému zlepšení držení a vnímání těla. V neposlední řadě došlo ke zlepšení jezdeckého sedu. Závěr: Koňský pohyb má prokazatelně pozitivní účinky na pohybový aparát jezdce, pokud je ale jezdec schopný odpružení koňského pohybu, tedy mít při každodenním tréninku nezávislý sed. Pokud sportovní jezdec stráví několik hodin denně v sedle a to navíc v nesprávném jezdeckém sedu, vliv koňského pohybu již zdaleka nemá pozitivní účinky. Pro jezdce není přínosem , ale pouze důvodem ke vzniku FPPS a chronické bolesti.

Abstract:

Aim: The aim of the thesis is to contribute to physiotherapy and sports riding problematics. Methods: This bachelor thesis was prepared as a case study. The case report of a 30 years old female sports rider, who has been examined for functional disorders of the musculoskeletal system due to excessive load, was observed. To elaborate the case report, observation, measurement and testing methods were used. To obtain the medical history and other important information about the patient's condition was used an interview method based on spoken communication. Another method which was chosen, was the observation method, which was used for aspects examination and for objective evaluation of individual therapies course. The measurement method was used to determine specific values and for this, the goniometer and a measuring tape were used. Based on this information, other special tests were executed and from the results of all examinations the physiotherapeutic plan was designed. The plan was realized through 10 visits during 8 weeks. In the therapy were used these methods and concepts - soft and mobilization techniques, post-isometric relaxation, reciprocal inhibition, stretching, kineziotaping, DNS elements (Dynamic Neuromuscular Stabilization), innovative Klapp climbing, senzomotorics and Centered Riding. The patient was intructed to do home therapy and to engage the therapy elements (torso stabilization) during horseback riding. In the middle of the therapies we performed a control examination focused on the main pathologies. To determine the long-term plan, the patient absolved an exit kinesiological examination. Results: During the therapy the pain was removed through the joint blockades release (SI joint) and thus the reflective changes dissapeared. The shortened cultures were also stretched - hip joint adductors and flexors. Breathing stereotype and deep stabilization system involvement were improved. Furthermore, the body posture and perception improved significantly. And finally, the riding position was improved. Conclusion: Horse movement has demonstrably positive effects on the rider's musculoskeletal system if the rider is able to compensate or cushion horse movement. Thus, the rider must have an independent (correct) sitting position during daily training. If the rider rides 6 or more horses in the long term and spends several hours a day in the saddle in the wrong riding position, the effect of horse movement has no longer positive but negative effects and it is not a benefit for the rider and it can cause FPPS and chronic pain.
