Women's satisfaction with the care of community midwife during the puerperium

Abstract:

Komunitní péče v porodní asistenci má své nezastupitelné místo v těhotenství, při porodu i v šestinedělí. Využívání služeb komunitní porodní asistentky v domácím prostředí není tak běžné, jako využívání těchto služeb v období těhotenství. Přitom právě šestinedělí je obdobím pro ženu a její rodinu velmi náročným jak po fyzické, tak i psychické stránce. Porodní asistentka je pro ženu vhodným a kompetentním rádcem, průvodcem a oporou v tomto období a využití jejich služeb v šestinedělí přináší ženě pocit bezpečí a jistoty. Cíl: Cílem této práce bylo zjistit a porovnat spokojenost žen v období šestinedělí s poskytovanou péčí komunitních porodních asistentek ze třech vybraných měst České republiky. Metodika: Výzkumný soubor tvořilo celkem 165 žen v období šestinedělí z vybraných měst, které využily služeb komunitní porodní asistentky v tomto období. Pro sběr dat byl použit standardizovaný dotazník spokojenosti pacientů s primární péčí EUROPEP, který bylo možné použít také pro tento výzkum. Tento dotazník byl rovněž doplněn o demografická data. Výzkumné šetření probíhalo od října 2018 do ledna 2019. Výsledky: Statisticky významný rozdíl ve spokojenosti žen byl zjištěn u tří z pěti domén. Jednalo se o domény Kvalita péče (p=0,0205), Organizace služeb (p=0,0439) a Dostupnost péče (p=0,003). V prvních dvou zmíněných doménách bylo nejlépe hodnoceno Město 1 a nejhůře Město 3. Ve třetí zmíněné doméně bylo nejlépe hodnoceno Město 2 a nejhůře opět Město 3. Při zkoumání nestandardizované části dotazníku byla zjištěna statistická významnost u závislosti věku a vzdělání na spokojenosti se službami komunitní porodní asistentky. Nejlépe tyto služby hodnotily ženy s vyšším a vysokoškolským vzděláním a primipary.

Abstract:

Community care in midwifery has an irreplaceable place in pregnancy, delivery and postpartum. The use of community midwife services at home is not as common as using these services during pregnancy. At the same time, the puerperium is a very demanding period for a woman and her family, both physically and mentally. The midwife is a good and competent counselor, guide and support for the woman, and the use of their services in the postpartum period brings a sense of safety and security. Aims: The aim of this work was to find out and compare the satisfaction of women in the postpartum period with the care of community midwives from three selected cities in the Czech Republic. Methods: The research group consisted of 165 women in the postpartum period from selected cities that used community midwife services during this period. For data collection, a standardized EUROPEP primary care satisfaction questionnaire was used, which could also be used for this research. This questionnaire was also supplemented with demographic data. The research started on October 2018 and ended on January 2019. Results: A statistically significant difference in female satisfaction was found in three of five domains. These were Domains of Care Quality (p=0.02), Organization of Services (p=0.04), and Care Availability (p<0.001). In the first two of these domains, City 1 and City 3 were the best rated. City 2 was the best rated in the third, and City 3 was the worst. In the non-standardized part of the questionnaire, statistical significance was found in the dependence of age and education on satisfaction with the services of a community midwife. Women with higher and tertiary education and primipara rated the services best.
