Experimental design of clinical trials in cardiology

Abstract:

Diplomová práce se zabývá problematikou experimentálního designu v oblasti klinického hodnocení léčiv, především možnostmi plánování a hodnocení třetí fáze klinického hodnocení léčiv. S použitím dostupné literatury jsou představena aktuální uspořádání klinických studií v kardiologii. Klinické studie slouží k testování nových léčivých přípravků, léčebných postupů a zdravotnických prostředků. Cílem klinických studií je zjistit účinnost a bezpečnost těchto léčebných postupů před zavedením do běžné klinické praxe. Úvodní část diplomové práce popisuje průběh klinického hodnocení a jednotlivé typy klinických studií. Jsou zde popsány a vysvětleny nezbytné předpoklady pro správné plánování klinického hodnocení s ohledem na předem stanovené cíle klinické studie. Dále jsou představeny typy designů charakteristické pro jednotlivé fáze klinického hodnocení, které zohledňují cíl klinického hodnocení a statistické zpracování vzhledem k hodnoceným parametrům. Závěr teoretické části je tvořen rozhodovacím schématem pro plánování třetí fáze klinických studií v kardiologii. Praktická část je zaměřena na stanovení hypotéz pro dané cíle klinických studií, vysvětlení návrhu použitého designu vzhledem k těmto cílům a výpočet optimálního počtu pacientů, kteří budou zařazeni do klinické studie. Pro dosažení statistické a klinické významnosti je volba optimálního počtu pacientů nezbytná. S použitím dostupných výpočetních programů je výpočet proveden pro různé typy hodnocených parametrů.

Abstract:

This paper deals with the experimental design in drug evaluation, especially with planning and evaluating of phase III of clinical investigation. Current organization of clinical trials in cardiology are presented in this thesis. using available literature. Clinical trials are used to evaluate new drugs, therapeutic procedures and medical devices. Objective of clinical trials is to determine efficiency and safety of the treatment before their introduction in clinical practice. The first part of the paper describes processes of clinical trials and their different types. Assumptions necessary for proper planning of clinical trials are described and explained here. The next part presents different types of designs that are typical for individual phases of clinical trials. Each of the designs reflects the objective of the clinical trial and statictical analysis in relation to assessed parameters. At the end of the theoretical part the decision diagram for planning the phase III of clinical trials in cardiology. The practical part is focused on formulation of the null hypothesis for given objectives of the clinical trials. Each example explains the use of designs in relation to their objectives and calculation of the number of patients who are enrolled in clinical trial. The optimal sample size is important to achieve statistical and clinical significance. The sample size calculation for different assessed parameters is calculated using available software.
