Studies of Occurence of ESBL Strains of Gramnegative Bacteria

Abstract:

Cílem této bakalářské práce byla analýza výskytu kmenů gramnegativních bakterií produkujících širokospektré beta-laktamázy ESBL (Extended-Spectrum Beta-Lactamases). Gramnegativní bakterie produkcí těchto enzymů se brání vůči působení antibiotik, proto v posledních čtyřiceti letech dochází k výrazné rezistenci bakterií k antibiotikům. Dnes je toto šíření bakteriální rezistence, mimo jiné způsobené ESBL kmeny, zásadním problémem nejen léčby infekci nemocničních, ale také v komunitních prostředích. V antimikrobiální léčbě je rezistence největším a stále rostoucím problémem. Práce shrnuje výskyt a také stanovení rezistence na antibiotika u kmenů Klebsiella pneumoniae produkujících ESBL na Třinecku a Frýdecko-Místecku s porovnáním na Ostravsku v roce 2009. Výskyt byl analyzován u mužů a žen hospitalizovaných v nemocnici nebo léčených praktickými lékaři. Dále pak bylo provedeno zastoupení počtu osob pozitivních na ESBL kmeny Klebsiella pneumoniae ve věkové kategorii a také bylo hodnoceno stanovení citlivosti bakterie na antibiotika (nitrofurantoin, gentamycin, kotrimoxazol, kyselina oxolinová). Výsledky práce svědčí o tom, že celkový průměr výskytu ESBL kmenů Klebsiella pneumoniae je 20,2 %. Průměrná rezistence na nitrofurantoin je 76,6 %, na gentamycin je 83,7 %, na kotrimoxazol je 96,3 % a na kyselinu oxolinovou je 87 %.

Abstract:

The aim of this bachelor thesis was the analysis of strains of gramnegative bacteria occurrence producing ESBL (Extended-Spectrum Beta-Lactamases). Due to production of these enzymes the gramnegative bacteria are prevented from the antibiotic effect, therefore there has been a significant resistance of bacteria to antibiotics in the last forty years. Today this spread of bacterial resistance, in particular caused by ESBL strains, is the fundamental problem of infection treatment not only in hospitals, but also in community environment. In the antimicrobial therapy the resistance is the greatest and ever growing problem. This work includes the occurrence and the antibiotic resistance determination in Klebsiella pneumoniae strains producing ESBL in Trinec and Frydek-Mistek in comparison to Ostrava in 2009. The antibiotic resistance had been analyzed in men and women hospitalized in hospital and in patients treated by general practitioners. Further a substitution analysis was made in the number of ESBL-positive strains Klebsiella pneumoniae persons of different age groups and also the assessment of bacteria sensitivity to antibiotics (nitrofurantoin, gentamicin, co-trimoxazole, oxolinic acid) was evaluated. Findings of this work make evidence that the overall average of occurrence of ESBL Klebsiella pneumoniae strains is 20,2 %. The average resistance to nitrofurantoin is 76,6 %, to gentamicin 83,7 %, to co-trimoxazole 96,3 % and to oxolinic acid is 87 %.
