Fast Track Technique in Intestinal Surgery

Abstract:

Cíl: Cílem diplomové práce bylo zjistit, zda metoda Fast Track snižuje pooperační komplikace, urychluje hospitalizaci a zvyšuje komfort pacientů po střevních operacích. Metodika: Průzkum byl realizován kvantitativní výzkumnou metodou za využití standardizovaného nástroje. Výzkumný soubor tvořilo 100 respondentů ve věku 65 až 90 let. Výsledky: Z celkového počtu 100 pacientů bylo 61 % můžu a 39 % žen. Celkem 62 % z nich podstoupilo operaci z důvodu karcinomu rekta. Operační výkon byl v 72 % proveden laparoskopickým přístupem. Nejvíce pacientů spadalo do klasifikace ASA III a to 53 %. Pooperační perorální příjem mělo 65 % respondentů za 7-12 hodin po operaci a obnovenou střevní peristaltiku mělo 48 % za 25-36 hodin od operačního výkonu. K vyprázdnění stolice došlo nejčastěji 4. pooperační den u 35 %. Pooperační nauzeou trpělo 59 % všech pacientů a odezněla u 41 % již 2. pooperační den. Mobilizace pacienta zahrnovala sed, kde byl průměr 15,51 hodin od operace a vertikalizaci, u které byl průměr 23,97 hodin. Pooperační komplikace byly u 26 % pacientů z celkového počtu. Permanentní močový katetr mělo zavedeno 80 % pacientů a k vytažení došlo nejčastěji 3. pooperační den. Břišní drén mělo zavedeno 55 % pacientů a odstraněn byl 4. pooperační den. Průměrná délka hospitalizace byla 9 dní a 87 % pacientů bylo propuštěno do domácí péče. Závěr: Metoda Fast Track urychlila hospitalizaci a zvýšila komfort u respondentů ve věku 65 až 71 let. Abstract:

Introduction: The aim of this diploma thesis is to find out, whether the Fast Track (also known as ERAS) reduces postoperative complications, shortens hospital stay and improves patient comfort after intestinal surgery. Methodology: The research was carried out using a quantitative research method using a standardized tool. The research group consisted of 100 respondents aged 65 to 90 years divided into two age groups, in which the evaluation factors were compared. Results: Of the total number of 100 patients, 61 % were men and 39 % were women. A total of 62 % of them underwent surgery for rectal cancer. The surgical procedure was performed in 72% of the procedures were carried out laparoscopically.. Most patients fell into the ASA III classification, 53 %. 65 % of respondents had postoperative oral intake 7-12 hours after surgery and 48% had restored intestinal peristalsis 25-36 hours after surgery. Stool emptying most often occurred on the 4th postoperative day in 35 %. Postoperative nausea affected 59 % of all patients and 41 % resolved on the second postoperative day. Patient mobilization included sedation, which averaged 15.51 hours postoperatively, and verticalization, which averaged 23.97 hours. Postoperative complications were present in 26 % of patients. 80 % of patients had a permanent urinary catheter inserted and the removal occurred most often on the 3rd postoperative day. Abdominal drain was introduced in 55 % of patients and removed on the 4th postoperative day. The average length of hospital stay was 9 days and 87 % of patients were discharged to home care. Conclusion: The Fast Track method accelerated hospitalization and increased comfort among respondents aged 65 to 71 years.
