Effect of obstetric injuries on women´s sexual life after childbirth

Abstract:

Cíl: Hlavním cílem práce bylo zjistit, zda porodní poranění ovlivňuje sexuální život žen po porodu. Byly stanoveny čtyři dílčí cíle, zda se liší vliv episiotomie a ruptury hráze, zda ženy s porodním poraněním a ženy bez něj začínají se sexuální aktivitou po porodu ve stejnou dobu, zda porodní poranění působí změny v sexuálním prožívání a zda má sutura poranění vliv na poporodní sexualitu. Metodika: V říjnu až prosinci 2013 byl proveden kvantitativní průzkum (dotazníkové šetření) v gynekologických ambulancích v Jihomoravském kraji. Do průzkumu bylo zařazeno 101 respondentek. 81 žen s porodním poraněním spadalo do výzkumného souboru a 20 žen bez poranění do kontrolního. Výsledky: Téměř polovina respondentek uvádí změny v sexuálním životě po porodu. Vliv episiotomie a ruptury je odlišný, po episiotomii zůstává sexuální život častěji nezměněn. Naopak po rupturách jsou frekventovanější negativní i pozitivní změny. Většina žen se vrací k sexuální aktivitě mezi šestinedělím a půl rokem po porodu. Sutura negativně ovlivnila sexuální život zejména po komplikovaném hojení. Závěr: Z výsledků vyplývá, že existuje vliv porodního poranění na sexualitu žen po porodu, mělo by tedy dojít k eliminaci vzniku poranění na minimum. Zda je příznivější vliv episiotomie či ruptury, nelze jednoznačně určit. Péče porodních asistentek by měla být v tomto směru velmi individuální. Zřetelný je i vliv sutury. Zejména po komplikovaném hojení, což značí potřebu kvalitnější péče o šití a edukace.

Abstract:

Aim: The main aim was to find out whether obstetric injury affects on women´s sexual life after childbirth. Four component aims were defined, whether effect of episiotomy and perineal tear are different, whether women with obstetric injury and women without it start with postpartum sexual life at the same time, whether obstetric injury causes changes in sexual experience and whether a suture of obstetric injury affects sexuality after childbirth. Methodics: The quantitative exploration (questionnaire investigation) was performed in gynaecological ambulances in the South Moravia Region in October - December 2013. 101 respondents were included in the research. 81 women with obstetric injury sloped into the investigative file and 20 women without injury into the control. Results: Almost half of the respondents stated changes in their sexual life after childbirth. The effect of episiotomy and perineal tears is different, sexual life more often remains the same after episiotomy. On the other side negative and positive changes are more frequent after perineal tears. Most women return to sexual activity between confinement and half year after childbirth. The suture negatively affects sexual life after complicated episiotomy healing mainly. Conclusion: The results show, that it exists effect of obstetric injury on women´s sexual life after childbirth. The creation of obstetric injury should be eliminated to the minimum. It couldn´t be conclusively said, whether the effect of episiotomy is better than of the rupture. The midwifery care should be very individual in this area. The effect of suture is distinctive. Mainly after complicated healing, it indicates a need for more quality care about suture and education.
