Intoxication eith HHC and Kratom from the Perspekctive of a Paramedic

Abstract:

Tato bakalářská práce se zabývá problematikou intoxikací látkami HHC a kratomem v kontextu přednemocniční neodkladné péče. Práce je rozdělena na teoretickou a praktickou část. Teoretická část se zaměřuje na charakteristiku obou látek, jejich farmakologické vlastnosti, mechanismus účinku a zdravotní rizika spojená s užíváním. Dále jsou popsány klinické projevy intoxikace, rozdíly mezi somatickými a psychiatrickými symptomy a obecné postupy jejich zvládání v rámci PNP. Součástí teoretické části je také přehled legislativního rámce, který upravuje nakládání s těmito látkami a kompetence zdravotnických záchranářů při poskytování péče intoxikovaným pacientům. Praktická část je založena na analýze pěti kazuistiy. Každý případ popisuje konkrétní intoxikaci HHC, kratomem nebo jejich kombinací a zahrnuje podrobný průběh zásahu, monitoraci životních funkcí, použitou léčbu a rozhodnutí o směrování pacienta. Na základě analýzy kazuistik práce formuluje doporučení pro zdravotnické záchranáře, zejména v oblasti diagnostiky dominantních symptomů, volby vhodného zdravotnického zařízení pro transport pacienta a farmakologické intervence. Výsledky této práce mohou přispět k lepšímu pochopení problematiky intoxikací těmito látkami v přednemocniční péči a podpořit tvorbu jednotných postupů pro efektivnější zvládání těchto stavů.

Abstract:

This bachelor's thesis addresses the issue of HHC and kratom intoxications in the context of pre-hospital emergency care. The thesis is divided into a theoretical and a practical part. The theoretical part focuses on the characteristics of both substances, their pharmacological properties, mechanism of action, and health risks associated with their use. It also describes the clinical manifestations of intoxication, differences between somatic and psychiatric symptoms, and general management strategies within pre-hospital emergency care. Additionally, this section includes an overview of the legal framework, which regulates the handling of these substances and defines the competencies of paramedics in providing care to intoxicated patients. The practical part is based on the analysis of five case studies. Each case details a specific intoxication involving HHC, kratom, or a combination of both and includes a detailed description of the intervention process, monitoring of vital signs, applied treatment, and decisions regarding patient transport. Based on the analysis of these cases, the thesis formulates recommendations for paramedics, particularly in the areas of diagnosing dominant symptoms, selecting the appropriate medical facility for patient transport, and pharmacological interventions. The findings of this thesis may contribute to a better understanding of HHC and kratom intoxications in pre-hospital care and support the development of standardized procedures for more effective management of these conditions.
