Recent-onset coxalgia: muscular strengthening vs static stretching in subjects aged from 25 to 40 practicing the martial art of Kung Fu.

Abstract:

The hip pain is the most common disease that can affect the hip joint in adults; it almost always depends on local unrest (fracture outcomes, impiengment femoro-acetabular overload, positions held, explosive movements etc ...The patient presents a typical localized pain in the groin and sometimes in the buttocks; it is common to radiation of pain along the anterior aspect of the thigh to the knee.As the origin of pain is essentially mechanical, the latter is caused by walking and by the joint movement in general , and is relieved by rest . Pain induced by the load causes claudication Escape: the patient tends to load on the limb a little sore, shortening the stance phase on the foot corresponding overloading and also the contralateral limb , causing a latter pain . The pain induced by movement causes a reflex contraction of the surrounding muscles , especially the muscles of the hip external rotators . It is observed , therefore a limitation of internal rotation and , more belatedly , also of the other movements , up to a point of important functional limitation. There are also further repercussions of the hip , such as calcifications muscle-tendon and the adaptation of the said articulation following a repeated sporting gesture and maintained positions , with consequent changes of articular standard . I decided to study and analyze the coxalgia within a sporting context distinct and particular: the martial art of Kung Fu. Within this framework , in fact , the posture is of fundamental importance , especially as regards the position of the legs . The even , whatever their setting ( front, side , semi -lateral , recessed , upwards , towards the bottom , on closing or opening ) , are the connection point between the upper body and the lower one, ensuring strength and stability of movement and preserving these features in other joints of the lower limbs (eg, knee, ankle , etc. ... ) . Given the complexity and particularity of Kung Fu , which is derived from the use of specific positions maintained over time and repeated use for combat , it can trigger a range of issues and impacts the hip joint , (such as deficiency of rotations RI and RE and asymmetry between the two limbs ) , I decided to analyze and , if possible, draw up an effective workout program in a separate age group in order to overcome this problem explained above .

Abstract:

La Coxalgia č la piů comune patologia che possa colpire l?articolazione d?anca nell'adulto; essa dipende quasi sempre da disordini locali (esiti di frattura ,impingement femoroacetabolare, sovraccarico,posizioni mantenute,movimenti esplosivi ecc?). Il paziente presenta un dolore tipico localizzato in sede inguinale e talvolta in sede glutea; č frequente l?irradiazione del dolore lungo la faccia anteriore della coscia fino al ginocchio. Poiché l'origine del dolore č essenzialmente meccanica, quest?ultimo č provocato dalla deambulazione e dal movimento articolare in genere, mentre viene alleviato dal riposo .Il dolore indotto dal carico determina una claudicazione di fuga: il paziente tende a caricare poco sull'arto dolente, accorciando la fase di appoggio sul piede corrispondente e sovraccaricando inoltre l?arto controlaterale,provocando un?algia di quest?ultimo. Il dolore indotto dal movimento provoca per via riflessa la contrattura della muscolatura circostante, soprattutto dei muscoli extrarotatori dell?anca. Si osserva dunque una limitazione della rotazione interna e, piů tardivamente, anche degli altri movimenti,fino ad un punto di importante limitazione funzionale. Sono presenti inoltre ulteriori ripercussioni a carico dell?anca,come ad esempio calcificazioni muscolo-tendinee e l?adattamento della suddetta articolazione a seguito di un ripetuto gesto sportivo e posizioni mantenute,con conseguenti modificazioni dei rapporti articolari standard. Ho deciso di studiare ed analizzare la Coxalgia all?interno di un contesto sportivo ben distinto e particolare: l?arte marziale del Kung Fu. All?interno di tale disciplina,infatti,la postura č di fondamentale importanza,soprattutto per quanto riguarda la posizione delle gambe. Le anche,qualsiasi sia la loro impostazione (frontale,laterale,semi-laterale,incassata,verso l?alto,verso in basso,in chiusura o in apertura),sono il punto di raccordo fra la parte superiore del corpo e quella inferiore,garantendo forza e stabilitŕ nei movimenti e preservando tali caratteristiche anche nelle altre articolazioni degli arti inferiori (es: ginocchio,caviglia ecc?). Vista la complessitŕ e particolaritŕ del Kung Fu,derivata dall?uso di specifiche posizioni mantenute nel tempo e ripetute usate per il combattimento, esso puň scatenare una serie di problematiche e ripercussioni sull?articolazione dell?anca, (come ad esempio deficit delle rotazioni RI e RE ed asimmetria tra i due arti), che ho deciso di analizzare e,se possibile,stilare un programma di allenamento efficace in una distinta fascia d?etŕ al fine di ovviare a tale problematica sopra descritta.
