Arrhythmia from the Paramedic's Perspective

Abstract:

Mezi nejčastější srdeční onemocnění patří poruchy srdečního rytmu neboli arytmie. Jejich příčinou je poškozené vedení elektrického impulsu nebo porucha jeho vzniku v srdci. Některé arytmie jsou nezávadné a bývají zachyceny pouze při dlouhodobém zaznamenávání EKG. Jiné arytmie mohou způsobovat obtěžující, a dokonce i život ohrožující příznaky a projevy. U zdravých lidí většinou nemají vážné důsledky. U pacientů s postižením srdce, například po infarktu myokardu, mohou být životu nebezpečné. Správná a včasná diagnostika a vhodně zvolený postup už během přednemocniční neodkladné péče jsou u pacienta důležitá pro jeho další kvalitu života. Proto je během vyšetřování pacienta nezbytnou součástí práce zdravotnického záchranáře monitorování EKG. Součástí této bakalářské práce je teoretická část popisující anatomii srdce, fyziologii srdce a krevní oběh a dále vznik a vedení vzruchu srdečním svalem na buněčné úrovni. Dále se věnuji problematice elektrokardiografie, jejího popisu a hodnocení elektrokardiogramu pomocí raftingu. Následně je práce zaměřena na arytmie, u kterých se zabývám výkladem jednotlivých typů, jejich příčinami, projevy a léčbou. Důležitou součástí je u každé arytmie výklad EKG a jeho odlišností od fyziologické podoby EKG křivky. Zároveň jsou tyto kapitoly doplněny o ilustrativní EKG záznamy. V rámci odborné části jsou předloženy tři kazuistiky pacientů transportovaných do nemocnice zdravotnickou záchrannou službou. V diskuzi je shrnuta práce zdravotnického záchranáře, jeho možnosti při monitoraci EKG a jsou diskutovány další studie týkající se této problematiky.

Abstract:

The most common type of heart disease is heart rhythm disorder, arrhythmia. This disorder is caused by a damaged conduction of an electrical impulse or a failure of its formation in the heart. Some arrhythmias are harmless and are usually detected only during long-term ECG recording. Other arrhythmias may cause bothersome signs and symptoms, sometimes even life-threatening ones. They do not cause serious consequences for most healthy people. However, they can be life-threatening in patients with heart disease, such as after a myocardial infarction. Early and accurate diagnosis and proper treatment during emergency pre-hospital care are important for the patient‘s further quality of life. Therefore, ECG monitoring is an essential part of the paramedic’s work during the patient’s examination. The theoretical part of this bachelor‘s thesis describes the anatomy of the heart, the physiology of the heart and blood circulation, as well as the formation and conduction of the signal through the heart muscle at cellular level. Furthermore, there is a description of electrocardiography and the evaluation of electrocardiogram using rafting method. Subsequently, this work is focused on the different types of arrhythmias, their causes, symptoms and treatment. An important part of their description is the interpretation of the ECG and its differences from the physiological form of the ECG curve. These chapters are supplemented with illustrative ECG recordings. In the special part of this work, three case reports on patients transported to the hospital by the emergency medical service are presented. The work of a paramedic is summarized in the discussion with possibilities in ECG monitoring and other studies related to this issue are discussed.
