Zvládání stresu u pacientů s idiopatickými střevními záněty

Stress Coping in Patients with Inflammatory Bowel Diseases

Abstract:

Cíle výzkumu a hypotézy: Cílem výzkumu je porovnat osobnostní charakteristiky a strategie zvládání stresu u pacientů s idiopatickými střevními záněty (z ang. inflammatory bowel disease, IBD) -Crohnova choroba (CD) a ulcerózní kolitida (UC) v souvislosti s fyziologickými teoriemi stresu, psychoneuroimunologií a psychoneuroendokrinologií. Hlavní hypotézou je, že pacienti s IBD vykazují vyšší míru neuroticismu a svědomitosti, používají více negativních copingových strategií a v důsledku toho zažívají více stresu. Metoda: Data byla sesbírána od 25 pacientů s IBD ve věku 20-45 let, komparační skupina byla tvořena párovým vyrovnáváním z hlediska věku a pohlaví. Obě skupiny obsahovaly 18 žen (68 %) a 7 mužů (32 %), věkový průměr pacientů byl 30,36 (SD=7,472) a respondentů v komparační skupině 29,36 (SD=7,348). V remisi bylo 13 pacientů (52 %), v relapsu 12 (44 %). Respondenti vyplňovali dotazníky NEO-FFI, SVF 78 a Stress Profile. Výsledky: Výsledky parametrických t-testů a neparametrických Mann-Whitney U testů potvrdily, že pacienti v relapsu i v remisi dosahují vyšších hodnot neuroticismu a využívají více negativních copingových strategií (konkrétně v subtestech Rezignace a Sebeobviňování). U pacientů v relapsu i v remisi míra neuroticismu spíše silně koreluje s využíváním negativních zvládacích strategií (ρ=0,692; p<0,001), v komparační skupině se tento vztah neprokázal. Respondenti v komparační skupině i pacienti v remisi jsou více extravertovaní. Rozdíly v otevřenosti ke zkušenosti, přívětivosti ani svědomitosti nebyly prokázány. Pacienti v relapsu zažívají vyšší úroveň stresu než respondenti v komparační skupině. Doporučení: Pro podrobnější popsání působení stresu na zdravotní stav pacientů s IBD by bylo třeba realizovat rozsáhlejší, ideálně longitudinální výzkum za spolupráce odborníků z oblasti medicíny i psychologie.

Abstract:

Aims and hypotheses: The aim of the study is to compare personality characteristics and coping strategies in patients with inflammatory bowel diseases (IBS) - Crohn’s disease (CD) and ulcerative colitis (UC) in the background of physiological theories of stress, psychoneuroimunology and psychoneuroendocrinology. The main hypotheses were that patients with IBD score higher in neuroticism and conscientiousness, use more negative coping strategies and consequently suffer from more stress. Methods: A group of 25 IBD patients aged 20 to 45 (M=30.36; SD=7.472) was investigated and compared to 25 respondents without the disease (Mean of age=29.36; SD=7.348). The groups were balanced with respect to age and sex. Both groups consisted of 18 women (68 %) and 7 men (32 %). There were 12 relapsed patients (48 %) and 13 patients in remission (52 %) in the group of IBD patients. The used questionnaires were NEO-FFI, SVF 78 and Stress Profile. Results: The results of parametric t-tests and non-parametric Mann Whitney U tests confirmed that pacients with active disease and in remission as well scored higher in neuroticism and used more negative coping strategies (specifically in Resignation and Self-accusation). At all the patients the neuroticism correlates with using negative coping strategies (ρ=0,692; p<0,001), but not at the group of non-patients. Non-patients and patients in remission are more extroverted. There were no significant differences found in openess to experience, agreeableness and conscientiousness. Relapsed patients suffer from stress more than non-patients. Recommendation: Further, at best longitudinal investigation of a team of experts in medicine and psychology is needed to be done to describe properly the effect of stress on health of IBD patients.
