Reimbursement of areola micropigmentation for people affected by breast cancer by health insurance companies

Abstract:

Tato diplomová práce se zabývá ekonomickým hodnocením zdravotnických výkonů. Řeší prokázání nákladové efektivity techniky micropigmentace prsních bradavek a dvorců za účelem zavedení hrazení tohoto výkonu pojišťovnami. Porovnávanou alternativou je výkon vytvoření nové prsní bradavky a prsního dvorce chirurgicky. Tento výkon je v Seznamu zdravotních výkonů definován pod číslem 61391. K porovnání byly stanoveny náklady zkoumaného výkonu díky rozhovoru s odborníkem na úhrady a užitek stanoven vlastním provedením šetření za použití BREAST-Q dotazníku. Náklady srovnávané alternativy byly stanoveny pomocí Seznamu zdravotních výkonů a Úhradové vyhlášky pro rok 2022 a její užitek díky kombinaci dat z BREAST-Q dotazníku a dat získaných z odborných studií. Výsledkem je prokázání vyšší nákladové efektivity v případě techniky micropigmentace než v případě srovnávané alternativy, prokázání sociální potřeby a vytvoření návrhu postupu pro zavedení hrazení zkoumaného výkonu zdravotními pojišťovnami v ČR.

Abstract:

This diploma thesis deals with the economic evaluation of medical interventions. Thesis demonstrate the cost-effectiveness of the nipples and areola micropigmentation technique in order to introduce the reimbursement of this procedure by insurance companies. The compared alternative is the procedure of creating a new nipple and areola surgically. This procedure is defined in the Seznam zdravotních výkonů (list of medical procedures) under number 61391. For the comparison, the cost of the investigated procedure was determined through an interview with a reimbursement expert and the benefit was determined by conducting the research using the BREAST-Q questionnaire. The cost of the compared alternative was determined using the Seznam zdravotních výkonů and Zdravotní vyhláška 2022 (reimbursement ordinance) and its utility through a combination of data from the BREAST-Q questionnaire and data obtained from expert studies. The result is the demonstration of higher cost-effectiveness in the case of the micropigmentation technique than in the case of the compared alternative, demonstration of the social need and a proposal of procedure for implementation of the reimbursement of the studied procedure by health insurance companies in the Czech Republic.
